1. To collect information/data on the identified learning gaps of learners on the

preferred modalities based on assessment during the 1st and 2nd Quarter;

2. To determine the activities/interventions of teachers in addressing the identified

learning gaps;

3. To identify the expected outputs and check the means of verification.
