Improved detection of lesions - the new Olympus EVIS LUCERA SPECTRUM


Flexible bronchoscopy has developed considerably in recent years, facilitating efficient and effective treatment of a variety of conditions. New products and techniques for interventional bronchoscopy, such as transbronchial needle aspiration (TBNA), endobronchial ultrasound (EBUS) and diathermy bronchoscopy, have ensured that this continuously evolving speciality is one of the most exciting in medicine today. Instrumentation design is of paramount importance to Olympus, balancing the requirements of patient tolerance, image quality and sample size. The research and development team is committed to a policy of continual improvement of the instrumentation portfolio, taking advantage of the most recent technological advances. The new Olympus EVIS LUCERA SPECTRUM video system exemplifies this philosophy, setting a new standard for endoscopic imaging, offering the 1080i HighDefinition Television (HDTV) standard, with four times the image resolution of traditional systems. In addition, two special light observation modes, Narrow Band Imaging (NBI) and Auto Fluorescence Imaging (AFI), dramatically improve the detection of subtle lesions.

The new Olympus SmoothShot Aspiration Needle for TBNA has been designed with scope compatibility in mind. The needle never extends from the distal end when being moved forwards or backwards within the scope channel. Similarly, careful thought has gone into the rigid section at the distal end, thus reducing the chance of the needle piercing the sheath.

With exceptional puncture performance, the SmoothShot ensures optimal results in TBNA. In addition, an optional side-hole design offers an increased cell yield. The needles are supplied ready to use with an integrated handle, are individually sterile wrapped in boxes of four, and are available in 13mm and 15mm needle lengths to suit individual preference.

Olympus EndoJaw Forceps


The Olympus EndoJaw single patient use biopsy forceps are available for 2.0mm and 2.8mm channel diameters. The unique SwingJaw cups enable biopsies to be taken easily when approaching difficult angles. In addition, Olympus offers a range of cytology brushes designed for optimal cell yield.

Olympus BF-UC260F-OL8 enhancing staging of the mediastinum

NBI utilises the inherent properties of short wavelength light to enhance the contrast between capillaries and the surrounding mucosa, improving visualisation of the mucosal surface and aiding diagnosis. The high-resolution CCD incorporated into the Olympus BF-6C260 bronchovideoscope ensures optimal image quality in both white light and NBI mode. Thanks to a dedicated high-sensitivity CCD, the Olympus BF-F260 autofluoresence bronchovideoscope can detect the weak autofluoresence generated by healthy tissue when excited by short wavelength light in AFI mode. Malignant lesions, on the other hand, have a much-reduced autofluorescence. By displaying this difference as distinct colour tones on the monitor, it is possible to distinguish between healthy and unhealthy tissue for the early detection of lesions.

Conventional Transbronchial Needle Aspiration (TBNA) is a 'hit and miss' procedure for staging the mediastinum in lung cancer, reflected in sensitivity and specificity that is far from optimal. However, the development by Olympus of an ultrasonic bronchoscope to allow visualisation of the needle during TBNA offers the prospect of overcoming the barriers that have previously restricted its acceptance. The new Olympus BF-UC260F-OL8 is purpose designed for visualisation of the target node and confirmation of needle placement in real time during TBNA procedures using ultrasound imaging. This new technique can allow investigation of the majority of

the middle mediastinum on an Outpatient basis, and thus avoid the need for mediastinoscopy. Features of the new BF-UC260F-OL8 ultrasonic bronchoscope include:
