 Visa application form filled out and signed.

 A colour photograph with white background not older than the last 6
months.

 Passport valid up to 3 months after the expiry of the visa and contains at
least two blank pages.

 Photocopies of the biometric page of the passport and all visas.

 Previous passports covering the last 7 years and certificate of movements
certificate issued by the Mugamma for the first time travellers. Or in case of
passport lost..

 Invitation letter filled by a Maltese host or resident of Malta, signed and legelized
by a notary public or a lawyer ( declaration of proof ) and a copy from the
residence / passport of the inviting person.

 Proof of accommodation or confirmed hotel booking in Malta.

 Certificate of employment or commercial registry & tax card or enrolment
certificate from school/university or pension statement.

 Up-to-date personal bank account statement for the last 6 months (Stamped
& in English).

 Flight booking indicating the complete itinerary of the tour. From:________to_________

 Travel medical insurance policy of a duration equal to that requested for

visa, valid for all the countries of the Schengen space, that covers the
medical expenses and of repatriation in case of accident or disease
(minimum cover: € 30.000).
