INSTRUCTIONS:

1. Prepare this report in at least three copies. Forward two copies to the District
Treasurer; keep triplicate for Church File.

2. 1st Quarter -Jan. – Mar. - Submit report not later than April 10.
2d Quarter -Apr. – Jun. - Submit report not later than July 10.
3d Quarter -Jul. – Sept. - Submit report not later than October 10.
4th Quarter -Oct. – Dec. - Submit report not later than January 10

3. District Treasurer shall consolidate/summarize all these reports and forward this
summary to National Headquarters via National Treasurer together with this
original copy of Form 14-a.

4. Use short (8 ½ x 11) bond paper.
