Greetings!

We, the Group 2 researchers from Grade 12-Amorsolo of Humanities and Social Sciences (HUMSS) at
Angelo L. Loyola Senior High School (ALLSHS), are currently conducting a research study entitled
"Commute Length and Scholastic Performance of Angelo L. Loyola Senior High School Students”
A standardized questionnaire will be used as an instrument in the said research, which has the following
objectives:

1. To determine the commute distance of selected students at ALLSHS,

2. To identify the substantial correlation of commute length and the academic performance of
students at ALLSHS, and
3. To provide suitable recommendations to the students who have low performance in accordance
with their commute length.

Because of this, the researchers are asking for your help in evaluating the attached questionnaire so that it
can be given to participants. Hence, we have attached here our standardized questionnaire for your
reference. We welcome your suggestions and comments on how to improve the instrument. Your positive
response will be highly appreciated. Thank you!

"Commute Length and Scholastic Performance of Angelo L. Loyola Senior High School Students”.

In this regard, we would like to ask for your permission to let us conduct a research study among all tracks
of both Grade 11 and Grade 12 on both academic and technical-vocational livelihood tracks, with a total
of 100 respondents, consisting of 50 male and 50 female participants. In addition, the objective of our
study is to determine the factors that affect the academic performance of students at ALLSHS in
accordance with their commute length. Lastly, all the data and information to be acquired would be solely
used for research and academic purposes only.
