Agreement

1. This document sets out your consent for the above-named organization and the organizations
listed in clause 6 to collect, use, store and transfer the Personal Data (as explained below) of
yourself and the Child you are responsible for (either as parent, legal guardian or responsible
relative) according to the terms below.

Personal Data

2. The personal data we intend to collect and use for the purposes in clause 3 include (in
relation to you and/or the Child): name, phone number, email address, residential address,
identity card and/or passport details, date of birth, age, health information, racial or ethnic
origin, religious beliefs, photo images, and videos (“the Data”).

Purpose of Collection

3. The Data collected will be used for the following purposes:

a. Processing the child’s registration into the Christian Holistic Child and Youth
Development Program and other related programs and services (the “Programs”).
b. Management, operation and maintenance of the Programs and the Child’s
participation in those programs and any related medical care.
c. Assisting affiliates’ development and production of a Child Sponsorship Packet and
publically accessible online profile for potential and eventual Sponsors and other
similar sponsor engagement material and advertising.
d. Research for program improvement including compiling and analyzing statistics,
keeping records of participation levels.
e. Prevention or detection of misuse, crime and fraud including investigation of
complaints and audits.
f. Disclosures as required by any applicable law, rules, regulations, codes or guidelines.
4. If you do not provide personal data to us which is marked as being obligatory/mandatory, we
may be unable to provide a place within the program for your child.

Persons with access

5. The Data may be provided to the organisation's personnel and, if the data is provided to such
other third parties, its third party service providers who reasonably require access for the
purposes in clause 3.
6. The Data may be transferred to the following affiliates (who may be outside of the jurisdiction
in which your Data was collected):
a. Compassion International, Inc. to the extent required for the purposes in clause 3.
b. Compassion Philippines to the extent required for the purposes in clause 3.
c. Other affiliates of Compassion International, Inc. for the identification and
engagement of potential Sponsors for the Programs.
d. A potential or eventual Sponsor for the purposes of facilitating and encouraging their
engagement in the Programs and communication with the Child.
7. The parties in clause 6 a)-c) are required to comply with the obligations of the Information
Security Manual developed by Compassion International, Inc.
8. The Data may also be provided by us and our affiliates to any law enforcement agencies,
regulatory bodies or similar in the respective jurisdiction where it is required for legal
compliance purposes.

Access to Personal Data

9. You may request access to the Data we hold about you or your Child and correction of any
inaccuracies.
