Survey Questionnaire On The Level of Math Anxiety Toward Solving Math Problems
Dear Respondents,
PURPOSE: This instrument aims to measure the level of math anxiety toward solving math
problems of first-year BSMAR- E students of VMA Global College and Training Centers, Inc. in
terms of Arithmetic, Algebra, Solid Mensuration, and Trigonometry.
Description: This survey questionnaire will consist of ten questions each in arithmetic, Algebra,
Solid Mensuration, and Trigonometry and uses a modified standardized questionnaire to
determine the level of math anxiety toward solving math problems of first-year BSMAR- E
students of VMA Global College and Training Center Inc.
Please take note that your participation in this survey is voluntary. You may withdraw if you
feel uncomfortable in the process of answering the questions. Rest assured that your responses
will be treated with utmost confidentially. No information that discloses your identity will be
released or published without your specific consent to the disclosure and only imperatively
