This MEMORANDUM OF AGREEMENT is executed and entered into this ______ day of

Organized and created under laws of the Republic of the Philippines, herein represented in this
act by the MUNICIPAL MAYOR, HANEYA THERESA YAP-CHIONG, hereinafter
referred to as the FIRST PARTY;

LGU-Plaridel, hereinafter referred to as the SECOND PARTY;

-WITNESSETH-

WHEREAS, the First PARTY is a Municipal Local Government Unit of Baliangao and a
service provider of Dangerous Drug Abuse Prevention and Treatment Program such as but not
limited to Community-Based Drug Rehabilitation Program (CBDRP);

WHEREAS, the FIRST PARTY is responsible for ensuring the health and well-being
of its constituents, particularly in addressing issues related to drug dependency;

WHEREAS, the SECOND PARTY possesses the necessary qualifications, expertise,

and experience to conduct drug dependency examinations;

WHEREAS, the FIRST PARTY desires to engage the services of the SECOND
PARTY to conduct drug dependency examinations within its jurisdiction;

NOW, THEREFORE, for and in consideration of the mutual promises and covenants

herein, the sufficiency of which is hereby acknowledged, the parties hereto agree as
follows:

A. The SECOND PARTY shall provide drug dependency examination services to

individuals referred by the Local Government Unit of Baliangao, in accordance with the terms
and conditions set forth in this Memorandum of Agreement (MOA).

B. The SECOND PARTY shall conduct thorough examinations and assessments to

determine the presence and extent of drug dependency among referred individuals. The
examinations shall be conducted in accordance with applicable laws, regulations, and ethical
standards.

C. The examinations shall be conducted at Municipal Health Office designated by the

Local Government Unit of Baliangao. The FIRST PARTY shall provide adequate facilities and
resources necessary for the conduct of the examinations.
 D.The SECOND PARTY shall maintain strict confidentiality with respect to all
information obtained during the course of the examinations, in accordance with applicable laws
and professional standards.
