SURVEY-QUESTIONNAIRE

Greetings!
We are currently conducting research entitled “LEVEL OF EFFECTIVENESS OF LUPONG
TAGAPAMAYAPA IN ALANGALANG, LEYTE”.
May you share time and energy to accomplish the survey questionnaire. Confidentiality of your
response is assured. Thank you very much.

Direction, please put a check the number (5) if your answer is Very Much Effective,
(4) if Effective, (3) Neither Effective nor Ineffective, (2) Moderately Effective and (1) if Not
Effective.

Part I. Level of Effectiveness of Lupong Tagapamayapa as perceived by the residents as


respondents in terms of mediation procedure.

1. Following receipt of the complaint for
mediation, the Lupon Chairman (Punong
Barangay) is mailing summons to parties within
the stipulated time frame of 1 to 3 days.

2. The case brought before the Lupon Chairman


(Punong Barangay) was resolved within the
allotted 15 days from the first meeting.

3. If the parties cannot reach the agreement within


the 15-day time limit, the Lupon Chairman
(Punong Barangay) will refer the case to Lupon
members for the creation and selection of
Pangkat ng Tagapagkasundo.

4. The Lupon Chairman (Punong Barangay) in our


barangay permits the complainant and
respondents to select their Pangkat ng
Tagapagkasundo from a list of Lupon members
who have been appointed.

5. In our barangay, in case of failure of settlement


before the Pangkat ng Tagapagkasundo within
the prescribed period of 45 days, the pangkat
will automatically issue a certificicate to file
action.
Part II. Level of Effectiveness of Lupong Tagapamayapa as perceived by the Lupon
members as respondents in terms of mediation procedures.

1. All written submissions will receive by the
Lupon Chairman. Grievances and put all
verbal complaints in writing to individuals.

2. During the proceeding our Lupon


Secretary records and takes down all of
the minutes.

3. Our barangay’s Lupon cases are resolved


in proper forum (Barangay Hall, Peoples
Hall).

4. Our Lupon members keep their


appointments on time. Deliberation and/or
hearings.

5. Our Lupon members are not concerned


about their appearance. Fee neither
before nor after the hearings.

Part III. Significant difference among the respondents in terms of mediation procedures.

continue to accommodate both parties’ concern.

its constituents. Regardless of age gender,
political allegiance, or socioeconomic class
resolution is possible.
