注册承诺书

Registration undertaking

本人将按照开学相关安排及时完成报到（注册）手续。现正进行在线注册，就相关事项承
诺如下：I will complete the check-in (Registration) procedures in time according to the relevant
arrangements for the opening of school. I am registering online and make the following
commitments on relevant matters：

二、承诺事项/Commitments
1.保证填写和在线注册的各项信息以及所提交的各项材料真实、准确、清晰； Ensure that
all information filled in and registered online and all materials submitted are true, accurate and
clear;
2.确保持其中一种以下种类的证件入境来华或在华居留许可
Ensure that one of the following documents is maintained to enter or stay in China:：
（1）中华人民共和国签证 PRC visa
（2）中华人民共和国外国人居留许可 Residence permit for foreigners of the PRC
（3）中华人民共和国外国人永久居留身份证 Permanent residence ID card of foreigners of
PRC
*根据政策规定，若持其他类型证件入境，学校将不予注册。 According to the policy, if you
enter the country with other types of certificates, the school will not register.
3.针对新生：除完成本在线注册外，到校后，本人将持申请材料原件前往招生办公室进行
核验。若未通过核验，将被取消注册或学习资格。 For freshmen: in addition to the online
registration, when reach school, you will go to the admission office with the original application
materials for verification. If you fail to pass the examination, you will be disqualified from
registration or study.
4.确保遵守中国的法律法规和大连医科大学的校纪校规。 Ensure compliance with Chinese
laws and regulations and the discipline and rules of Dalian Medical University.

若违反以上情形，本人自愿承担相应后果。特此承诺。 In case of violation of the above

circumstances, I am willing to bear the corresponding consequences. We hereby promise.
