    WORK IMMERSION PARENTAL CONSENT

    This is to certify that I am allowing my son/S/daughter


    ____________________________________to undergo a Work Immersion for a minimum of
    80 hours or 10 days starting March 25 to 27, 2024 and April 1 to 11, 2024 as one of the
    subjects/requirements in Senior High School program of the K to 12 curriculum.
    It is understood that he/she abides the Occupational Health and Safety procedure and
    regulations that may be imposed by the Industry Partners and the School for his/her welfare
    and safety.
    I fully agree to waive any responsibility on the part of the School in case of any
    untoward incident that may happen to my son/daughter during the duration of the immersion.
