Pre-Employment Requirements

Welcome! Before you start your career journey, please complete the following pre employment
requirements based on the timeline below:

PRE-EMPLOYMENT CHECKLIST OF REQUIREMENTS FOR NEW HIRES


It is a must for you to complete these requirements before reporting for work as these will be critical to
employee information that will be encoded in our employee database.
MEDICAL EXAMINATIONS
1. Please go to our official doctors for your medical exam.

2. The clinic would prefer for you to come at 8:30 AM and present 2 valid IDs for authentication. The
medical examination will only take about three (3) hours.
3. The clinic will no longer accept newcomers by 3:00 PM.
4. Bring your stool specimen and give this to the clinic receptionist. The specimen stool should be
submitted to the clinic within two hours after collection. Urine specimen will be collected at
the clinic.
5. You need NOT follow up the results of your exam as they will send the results directly to HR
Office.
6. You need NOT pay any fee to _____. The firm will cover the expenses for your medical exam.
7. Fasten the certification slip signed by ________ (or custody and control form) in your pre
employment requirements folder.
8. For Female Staff: If you are currently having your monthly period, you may still proceed to the
clinic for the medical exam (except for fecalysis and urinalysis) and the staff will advise you
what day you should take the pending medical procedures.
9. Take note visitors/patients must always WEAR FACE MASK completely covering nose and mouth
(no valves). Wearing of both FACE SHIELD & FACE MASK is highly encouraged.

Medical Procedures to be conducted:


1. Urinalysis
2. Fecalysis
3. Physical Exam
4. X Ray
5. CBC
6. Drug Test

NOTE: Talent Team will only be your point of contact for all medical-related concerns. You are not
allowed to provide any confidential information about you and the firm. All
communications and transactions will be coursed between the Talent Team and the staff.

OTHER DOCUMENTS TO BE SUBMITTED


1. Transcript of Records (photocopy). The one that you used for the board exams will do.

2. Certification from the school for obtaining a Latin Honor (Summa Cum Laude, Magna Cum Laude,
Cum Laude; Honorable Mention or With Academic Distinction) if not indicated in the transcript
and only if applicable.

3. NBI Clearance - photocopy of the PERSONAL COPY or the original NOT VALID FOR TRAVEL
ABROAD copy secured within the past 12 months (NOTE: validity should cover your
probation period which is 6 months of your start date of work)
4. Requirements for Payroll Account Opening:
a. Fill-up the Bank Form (7pages) included in the Zip Folder. Submit the
accomplished form thru Onboarding email _____

b. Provide softcopies of 2 Government-issued ID (e.g. Passport, Driver’s License, PRC


ID,
Postal ID, Voter’s ID, Company ID, UMID, TIN ID, SSS Card and PhilHealth ID,
NBI Clearance).
c. Please wait for the reply of HR Onboarding Team for further instructions
once the bank form was verified by Onboarding team.
d. For the meantime please provide your personal savings account for the
crediting of your salary (only accepted banks are the ff: BDO, BPI (not BPI
Family Savings), Metrobank, Eastwest, RCBC, UCPB, Unionbank, Landbank).
Those with no personal savings account, you may temporarily use your
relatives personal account details. (provide the following: Full Name, Bank and
Personal Savings Account Number). Please be advised that this is a
temporary
arrangement while we process your payroll accounts with Eastwest to
avoid delays in receiving your salary.

NOTE: After filling out the bank account form and attaching the necessary requirements needed for
payroll account opening, please submit your form to the HR Receptionist. Failure to submit a
complete set of form on-time would mean a delay in the credit of your salary. Make sure that all
necessary fields are filled out as incomplete forms will lead to HOLD PAY. Please submit the bank
form immediately to Talent Admin Team on or before your 1st day.

5. Tax Identification Number (TIN)


a. New hires who do not have a Tax Identification Number (TIN), please fill-up the
BIR 1902 form (attached on the checklist in the email/ required fields need to fill up are
Nos. 6,7,8,9,10,11,12,13,15,22,32,39 ONLY) and send it to the HR Onboarding Team.
Attach a photocopy of Birth Certificate.
b. New hires who already have a TIN need to secure the following forms from HR:
1. TIN ID Card
2. e-REG page from the BIR Portal
3. BIR forms like 1901, 1902 and 1904
4. BIR Form 1905- Application for Registration Information Update – You will be the
one to file this in your current Regional District Office (RDO). New RDO office
should be under your permanent residence address. Once filed, please fasten a
copy in 201 folder.

6. SSS E-1 Form (photocopy) containing the SSS Number. This can be obtained from either of the
following SSS offices or locations:
OPTION 1:

• SSS branch in your locality.

• You may also secure your SSS Number at the following SSS branches in

Makati: o BUENDIA Branch: 355 ECC Bldg., Sen. Gil Puyat Ave., Makati City

OPTION 2:

• Online Registration (see document in the folder for SSS online registration
instructions).

NOTE: If you have an existing SSS number, you can provide your E1 Form (photocopy
only) or any updated verification slip from any SSS satellite branch.

• Please bring a copy of your SSS E-4 Form if you reported an additional dependent.

7. Three (3) Referral Letters addressed to Partner-In-Charge of Talent Team, Julie Christine O.
Mateo. There would be no specific format for the letter. Please ensure that the person will
include his/her signature and contact information (e.g., email address, contact number).

8. Three (3) photocopies of your Birth Certificate.

9. Three (3) pieces of 2x2 and three (3) pieces of 1x1 photos in WHITE background. Send it
thru Onboarding email.

10. PhilHealth Number

a. New hires who do not have a Philhealth Number yet:


1. Accomplish the forms provided (2 PMRFs & 1 ER2).

2. Go to any Philhealth branch and submit your accomplished forms. Once filed, you will
be given a Member’s Data Record (MDR). Your MDR should reflect ____ as your
employer.
3. Fasten the photocopy of your MDR in your pre-employment requirements folder.

b. New hires who do already have a Philhealth Number:


1. Accomplish the forms provided (2 PMRFs & 1 ER2), write your Philhealth number in
the boxes on the upper right side of the PMRF and check the “For Updating” box.
2. Go to any Philhealth branch and submit your accomplished forms. Once filed, you will
be given a Member’s Data Record (MDR). Your MDR should reflect _____ as your
new employer.
3. Fasten the MDR in your pre-employment requirements folder.

NOTE: Please indicate the date when you signed your contract under the column of “Date of
Employment” in the ER2 Form. Since PhilHealth does not accept future dates for the registration
of PhilHealth number.

c. New hires with dependents:


1. Accomplish the forms provided (2 PMRFs & 1 ER2) and attach the necessary
documents (please refer to the back portion of PMRF).
2. Go to any Philhealth branch and submit your accomplished forms. Once filed, you will
be given a Member’s Data Record (MDR). Your MDR should reflect ____ as your
employer.
3. Fasten the MDR in your pre-employment requirements folder.

11. Pag-Ibig MID Number:

Registration is online. Please see document in the folder for Pag-Ibig online registration
instructions. Once registered, submit printed copy of Member Data Form (photocopy) with
Pag IBIG MID number.
NOTE:

• Please make sure that the MDF’s EMPLOYER/BUSINESS NAME is updated under ______
• If you have an existing PAG-IBIG Number, you may proceed to any PAG-IBIG offices to
update your details.

12. Employees with Dependents

• Submit a photocopy of your spouse’s birth certificate, marriage certificate and


your child/children’s birth certificate for the enrollment of Medical Insurance.

• For Managerial Positions: Qualified managers are Single managers with “No Declared
Dependent/s” and may enroll one parent. Parent must be 65 years old and below.
**Please send the Birth Certificate of Qualified dependents to Talent Team Onboarding.

NOTES:
1. Existing SSS and/or HDMF Loan (for experienced hires) – Submit Loan Voucher,
Certificate of Amortization and Statement of Account directly to: Finance Department (

2. Always keep a copy of your Statutory Government numbers (TIN, SSS, PHIC, HDMF).

MANDATORY REQUIREMENTS for Professionals (CPAs, Lawyers, Nurses, Engineers, etc.)


For Certified Public Accountants -Submit two (2) copies each of the following:

• Photocopy of Board Rating

• Photocopy of PRC Certificate

• Photocopy of CPA License Card

• Professional Tax Receipt form Makati City Hall

For Lawyers – Submit two (2) copies of each of the following:


• Photocopy of Bar Rating

• Photocopy of IBP Card

• Certificate of Membership to the Bar

• Professional Tax Receipt form Makati City Hall

ADDITIONAL REQUIREMENTS FOR EXPERIENCED HIRES

• BIR Form 2316 or W-2 from previous employer. Pursuant to Section 9 of Revenue Regulations
No. 4-93 as BIR finally implemented R.A. 7497 on final Withholding Tax on Purely
compensation income, under the topic “Multiple Employment Successively”, if the employee
transferred to another employer during the taxable year, said employee shall furnish his new
employer with a Certificate of Compensation and Exemption (W-4) indicating his/her
previous employment during the taxable year and attach to the said certificate (W-4) the
Certificate of Income Tax Withheld on Compensation (W-2) for the calendar year issued by
previous employer/s.
• BIR Form 1905 – Update of marital status and personal details in BIR Records. Documents
needed for Tax Exemption, if applicable:
- Marriage Certificate
- Birth Certificate of Children

• Certificate of Employment and Clearance from the previous employer

13. Please also fill-up the Personal Information Update (PIU) and the Data Privacy Act. Kindly
read carefully all the details.

IMPORTANT NOTE: Always secure a copy of ALL your statutory numbers (please always keep a copy
of your statutory numbers on hand) together with the forms since HR will only be accepting
photocopies of all requirements for filing in your 201 folder in case you will be needing it for emergency
and personal matters. Please be guided accordingly. Thank you.

PLEASE SUBMIT ALL YOUR STATUTORY NUMBERS (TIN, SSS, PHIC, HDMF) VIA EMAIL WITH PROOF
OF DOCUMENT TO AVOID DELAYS IN REMITTING YOUR CONTRIBUTION IN RESPECTIVE
GOVERNMENT OFFICES.

SUBMIT ALL AVAILABLE SCANNED COPIES OF YOUR REQUIREMENTS ON YOUR FIRST DAY IN
