Task 3

This is an incident report for Mr. Gary Saunders , a male patient of Mount Royal Hospital. On
June 29, 2017, at 1:05 pm . The patient was found in his bathroom lying on the floor with
bleeding on his head. No witnesses was present when the incident occurred.

Mr.Saunders was found in the bathroom with a lacerated wound on his head. A clean dressing
was put on the patient’s head to minimize the bleeding. Vital signs were taken as follows; BP :
160/102 RR: 18 PR: 110. Mental assessment was done and the patient was conscious, coherent
and able to narrate what happened. According to the patient , he was preparing for his bath when
he slipped on the wet floor and struck his head. The client’s behavior and previous medication
which he had taken hours ago may have contributed to the incident. Patient persistently
complained of pain in his head and insisted to be brought back to his bed. Patients was then
assisted. The nurse raised the side rails to prevent another fall from re occurring.

The doctor was notified and ordered to prepare a suture for the possibility of suturing the cut.
The physician also ordered for an xray to rule out any possible skull fracture. An ice pack was
then applied to minimized the bleeding . The supervising manager and next of kin was been also
notified at the same time.

Task 4

This is a case of Mrs. Elizabeth Newman , a resident of Forest Hill retirement home . On March
9, 2021at 1:30 pm, the patient was found in the hallway agitated and confused. The husband was
on the scene when the incident happened .

Patient Newman was seen the hallway crying in pain, clenching to his chest. Upon arriving, the
nurse assessed the patient and take head to toe assessment. Vital Signs were taken as follows:
bp:160/102 RR:18 PR:110. The nurse also noticed the patient was showing signs of confusion
and persistently complaining of unbearable chest pain and difficulty breathing. It was believed
that the patient sustained a possible rib fracture. As verbalized by the patient, as she was walking
in the hallway, another patient who suffers from dementia and show signs of paranoria struck
her in the chest which led to her injury. Patient was then transferred to her bed and assisted on
her needs.

The attending doctor and the head nurse was informed regarding the incident . An ice pack was
placed to prevent further bruising. Tyelenol was also given every 6 hours to relieve pain as per
doctors order. Patient was sent to xray to confirm the diagnosis.
