LETTER OF INTENT

Dear Ma’am Ruth,

I hope this letter finds you in good health and high spirits. I am writing to
formally request a personal leave of absence from my duties as a TEACHER I
at LEOPOLDO D. DACERA SR. INTEGRATED SCHOOL for a period of 90
days starting from September 1, 2023 to November 30, 2023.

I deeply understand the importance of my role and responsibilities in the

school, and I assure you that this decision to seek personal leave has not
been taken lightly. Due to unforeseen circumstances in my personal life, I find
it necessary to take some time off to attend to urgent matters that require my
immediate attention. During my absence, I will do my best to ensure a smooth
transition and handover of my tasks to a suitable colleague or provide
comprehensive guidelines for their temporary assignment.

I believe that this temporary leave will allow me to address the personal
matters at hand, which will ultimately enable me to return with a renewed
sense of focus and dedication to my role within the school community. I am
committed to ensuring that my absence will have minimal disruption to the
school's operations and will diligently adhere to any guidelines or protocols set
forth by the school administration.

I kindly request your understanding and approval of this personal leave

application. Rest assured that I will be reachable in case of any urgent
matters that may arise during my absence. I have informed my immediate
supervisor, Fredgie P. Pasco, Ed.D., and my principal, Rob H. Masalon, about
this leave request, and they are fully aware and supportive of my decision.

I will be more than willing to provide any further information or documentation

necessary to process this leave application promptly. Please let me know if
there are any specific procedures I should follow or if there are any additional
requirements to facilitate the smooth approval of this request.

Thank you for considering my application for personal leave. Your support
and consideration during this time are greatly appreciated. I look forward to
returning to my responsibilities at Leopoldo D. Dacera Sr. Integrated School
after the leave period and continue contributing to the growth and success of
our school community.
