Background
As part of my Comprehensive Public Health Practicum at Health Office, Kathmandu, I was
assigned a field visit to private health institutions of ward no. 31 and 32 of Kathmandu
Metropolitan from 2080/10/05 - 2080/10/08.

Objectives:
i. To assess the current status of recording and reporting in the selected private health institutions
ii. To identify the problems related to recording and reporting in the selected private health
institutions

Methodology:
By reviewing list of private health institutions and their monthly reporting status in FY 2080/81,
the health institutions with reporting issues were identified and listed.

A purpose made questionnaire was prepared to collect information on basic information,

recording methods, reporting status, issues and recommendation.

A visit to selected private health institutions was made. Staff involved in recording and reporting
were interviewed to collect necessary information.

Key findings

i. DHIS 2 access and use: None of the institutions currently used DHIS for online
recording and reporting. The reasons were; problem with previous ID, delay in getting
DHIS ID after application and not having applied for DHIS ID.

ii. Recording : The private institutions did not use any standard tools for daily patient
recording. They used registers as per need and service for daily recording.

iii. Reporting: All institutions filled monthly HMIS reporting form. Some submitted
monthly report to Kathmandu Metro Health Division as per new system. Some
institutions reported to Health Office, Kathmandu.

iv. Training for Recording and Reporting: All visited private health institutions lacked
formally trained manpower for recording and reporting.

Problems:

i. Health Office did not maintain monthly reports submitted.

ii. There is delay in getting DHIS 2 ID.
iii. Issue with DHIS 2 entry were not addressed.
iv. The forms are too vast for institutions that provide small number of services.

Recommendations:

i. DHIS 2 ID should be made available for easier recording and reporting.

ii. Training on recording and reporting tools is needed.
iii. Monthly reporting using DHIS 2 only without hardcopy would be easier.
