Hundered (300) square meters and Lot No. ____ covered by TCT No.
T-____containing an area of Two Hundred Nineteen (219) square
meters situated at _____ and is affected by Road Widening Project of
Department of Public Works and Highways (DPWH) - 2nd District
Engineering Office (DEO);

3. That as an owner I actually and personally in possession of the said

building/s since several years ago up to the present;

4. That, I am executing this affidavit to attest the truth of the foregoing

facts and likewise no malice or fraud intent; and

5. That, I have therefore executed this affidavit to confirm the truthfulness

of the foregoing and for whatever legal purposes this Affidavit may
serve.
