AFFIDAVIT OF LOSS

I, JONARD DALAGAN GASPAR, of legal age, single, Filipino citizen and a

resident of Alyangan Drive, Mampang, Zamboanga City, Philippines, after having been
duly sworn to in accordance with law, hereby depose and say, that:

1. I am an employee of Microwide Distributor Center with business address at

Tumaga, Zamboanga City and as such I was issued a Company Identification
Card (ID). Said Identification Card was usually kept in my wallet.

2. Some time in the last week of the month of June 2009, due to inadvertence
my wallet got lost including my Identification Card;

3. I exerted diligent efforts to locate and to look for the above-mentioned

Identification Card but the same proved futile;

4. For all intents and purposes, I am now declaring the above-mentioned

Identification Card lost and beyond recovery;

5. I execute this Affidavit to attest to the truth of the foregoing facts based on my
personal knowledge and authentic records and for whatever legal purpose it
may serve.
