THIS IS AN ATTEMPT TO COLLECT A DEBT.

AMOUNT DUE: P135, 000.00

YOU HAVE 15 DAYS TO PAY ENDING ON OCTOBER 25, 2023

Dear Coronel,

You currently have an outstanding balance with interest in the amount of One Hundred Thirty Five
Thousand Pesos (P135, 000.00).

The outstanding balance is in regard to the salary loan agreement you have entered with a Principal
amount of One Hundred Fifty Thousand pesos (P150, 000.00) with a monthly interest rate of
5% resulting to 20 equal continuous semi-monthly installments of P11, 250.00 each on the 15 th
day and 30th day of each month dated October 28, 2021.

However you only pay a total of 8 times in 2022 and lastly February 2023 that causes a delay and
failure to pay in your long outstanding obligation.

In order to resolve this matter, you shall have fifteen (15) days to pay this debt.

If there is no response to this letter, all legal rights shall be explored, including, but not limited to,
legal proceedings necessary to recover the debt without further notice.

This demand letter for payment serves as official notice to you and may be tendered in court as
evidence of your failure to pay. If legal action is to occur to resolve this matter, it may involve
having you pay attorney’s fees and may impact your credit history.

I hope to resolve this matter as soon as possible.
