• If you pass through border control with a minor, you must be able to show that you have the consent of the child’s
parent(s) or guardian(s) who are not travelling with the child.
• You can use this form to show this. Your passage through border control will be quicker if you have the form with you.
• Complete a copy of this form for each child. After filling in the form, print and sign it.
