2021 (Inheritance)
1. Municipal Agrarian Reform Program Officer (MARPO) Certification (LTC Form No. 2)

2. Duly Notarized Application for Issuance of Land Transfer Certificate (LTC Form No. 1).
(Notaryadong kahilingan na mabigyan ng Land Transfer Certificate na nilagdaan ng mga
TAGAPAGMANA)

3. Original/Certified Photo Copy of Extrajudicial Partition to be registered. (Pinatunayang

Tunay na Kopya ng Kasulatan ng Pagmamanahan na nais iparehistro).

4. Electronic Copy of Title. 6 months validity from the date of issuance. (Pinatunayang Tunay
na Kopya ng Titulo. Hindi lalampas ng anim na buwan simula nang ito ay makuha).

5.Certified Copy of Tax Declaration. 6 months validity from the date of issuance.
(Pinatunayang Tunay na Kopya ng pahayag sa pamumuwisan ng lupa. Hindi lalampas ng 6
na buwan simula nang ito ay makuha).

6. Affidavit of the TRANSFEREE/HEIRs stating that the aggregate landholding and the subject
property/properties is/are retention or portion of the retention area. (Sinumpaang Salaysay
ng TAGAPAGMANA, na ang kaniyang kabuuang pag- aaring lupang agrikultural at ang lupa
nabanggit ay bahaging kaniya/kanilang retention).

7. Certification to be issued by the City/Municipal Assessor and Provincial Assessor’s Office,

regarding the extent of agricultural landholding where the property is located and/or where
the REGISTERED OWNER and HEIR reside. (Pagpapatunay ng Tanggapan ng Munisipal
Pie. Ac, at Panlalawigang Taga-tasa ukol sa lawak ng pag-aaring lupang agricultural kung saan
matatagpuan ang lupa at lugar kung saan nakatira ang NAKATALANG MAY-ARI NG LUPA at
TAGAPAGMANA).

8. Copy of Death Certificate of the deceased landowner). (Katibayan ng pagkamatay ng

may- ari ng lupa).

9. Original/Certified Photo Copy of Secretary’s Certificate/Board Resolution, in case the one of

the party is a corporation. (Pagpapatunay ng Kalihim/ Pinagtibay na Resolusyon ng mga
miyembro/ director ng korporasyon, kung ang isang panig ay korporasyon).
10. Special Power of Attorney, if the applicant is not one of the transferor or transferee.
(Natatanging gawad kapangyarihan kung ang nag- aayos ng aplikasyon ay hind isa sa may-
ari ng lupa o paglilipatan).

12. Valid IDs of heirs (IDs ng mga tagapagmana)

13. Original copy of Affidavit by the farmer/tenant tilling the subject land that he/she was
informed of the transfer of ownership. (Sinumpaang Salaysay ng magsasaka/tenante na siya
ay nabigyan ng patanto ukol sa paglilipat ng pagmamay-ari ng lupa)

14, copy of Order of approved transfer if the Transfer Certificate of Title (TCT) is originally an
awarded land that is CLOA or EP. (Kopya ng pinagtibay na pagsasalin ng pagmamay-ari kung
ang Titulo ay nagmula sa CLOA 0 EP).

15.Copy of birth certificate of transferee if the transaction is Deed of Self-Adjudication. (Kopya

ng birth certificate ng paglilipatan kung ang dokumento ng paglilipat ay Deed of Self
Adjudication).
