CONTRACT OF LEASE (EXTENSION)

KNOW ALL MEN BY THESE PRESENTS:

This Contract of Lease made and entered into this ______ day of ___________, at City of
__Mandaluyong__, by and between:

Michael A. Mahusay of legal age, Filipino , with principal address at Unit

1117 Tower 4 ,Avida Towers Centera Brgy .Highway Hills Mandaluyong
City ,hereinafter referred to as the “LESSOR”;

-- and –

Michaela T. Mensalvas, of legal age, Filipino, and with principal address at #104 Gloria
Teresa St. Villa Gloria Subd Angeles City Pampanga, hereinafter referred to as the
“LESSEE”.

W I T N E S S E T H that:

WHEREAS, the LESSOR is registered, legal and absolute owner of that SEMI FURNISHED
RESIDENTIAL UNIT known as 11TH FLOOR, UNIT 1117, TOWER 4, AVIDA TOWERS
CENTERA located at EDSA Cor. Reliance St., Mandaluyong City, Philippines, hereinafter referred to as
the “LEASED PREMISES”.

WHEREAS, the LESSEE desires to lease the above-mentioned LEASED PREMISES and the
LESSOR is willing to lease the same unto the LESSE, subject to the terms and conditions hereinafter
specified.

NOW, THEREFORE, for and in consideration of the foregoing and mutual covenants
hereinafter contained, the LESSOR has let and by these presents does hereby let and lease unto the
LESSEE the aforesaid LEASED PREMISES and the LESSEE hereby accepts the same by way of lease,
subject to the following terms and conditions:

• PERIOD OF LEASE EXTENSION - This lease shall be for a period of three (3) months
extension commencing from MAY 15, 2024 to MAY 14, 2025 renewable under such terms
and conditions by mutual agreement of both parties, in which case, the LESSEE must signify
his intention to extend by giving a written notice thereof to the LESSOR at least sixty (60)
days prior to the expiration of this contract.

• AMOUNT OF RENT/ DUES – The parties herein agreed that the monthly rental of the
LEASED PREMISES shall be TWELVE FIVE HUNDRED THOUSAND PESOS ONLY
(₱12,500.00) Philippine Currency, Inclusive of Association Dues.

• THE OLD/PREVIOUS CONTRACT ATTACHED (2021 TO 2022) – Shall serve as the

reference/basis of the extension of this contract.

IN WITNESS WHEREOF, the parties have hereunto set their hands at the place and date first
above written.
