RE: WARNING LETTER

We hereby write to you to inform you that Midway’s Tavern Limited is issuing you this letter for the
following transgressions;
- Not being observant to your workstation.
- General tardiness at work.

The above acts, as noticed to have been committed by you, amount to misconduct which, if proved
by the Directors would warrant serious disciplinary actions against you.

Accordingly, you are hereby required to change your cause of actions to adhere to the rules and
regulations and also to observe time during your working hours as per the terms you were given.

Should you fail to concur with the guidelines as required, it will be presumed that you are not fit for
the position affirmed in, and this will lead to summary dismissal from Midways Management.

The receipt of this letter should be acknowledged.
