January B777 MAA - DXB Pushback


2024 HAZARDOUS AIRCRAFT PUSHBACK - WINGTIP CLEARANCE
Cleared to pushback from stand 27 onto taxiway D1 then to pull onto taxiway H, abeam stand 28. The flight crew noticed the tug
operator initiated the turn slightly inside the taxi line and the engineer's position would not allow him to have a clear view of the
right wing. Engineer was asked whether wing walkers were present and replied no. The flight crew was concerned with the wingtip
clearance with OAL parked on bay 49 and instructed the engineer to stop the push back till a wing walker was present. The aircraft
was pulled forward with oversteer onto the correct position on taxiway H.
Flight Safety update: The report has been forwarded to MAA EKAS and designated Engineering Regional Manager for investigation
and follow up.

December B777 DXB - MAA BLR Approach


2023 DESTABILISED APPROACH - SPEED - GO-AROUND AND DIVERSION
During the ILS approach to runway 25, a go-around was flown when the approach became destabilised for speed due to variable
winds and a drop in visibility due to heavy rain.
Flight Safety update: The QAR data indicate crosswind from 180 degrees at 45 knots at 600 feet RA. Within a span of six seconds,
the headwind component shifted from seven knots to 25 knots, causing instability in the approach speed. Because of heavy rain
and the potential for a contaminated runway, the flight crew performed a standard go-around from 377 feet RA. After conducting a
weather analysis, the flight diverted to its alternate.

2023 LIGHTNING STRIKE - NO DAMAGE
During weather avoidance on departure out of Chennai aircraft got struck by lightning at the radome.
Flight Safety Update: an engineering check completed and no damage found.

September B777 DXB - MAA Approach


2023 GO AROUND - ATC SERVICE STANDARD - TAILWIND
Upon arrival at VOMM, the aircraft was rerouted to runway 07 due to thunderstorms north of the airport. ATC was busy, causing
delays in descent clearance. During the approach to runway 07, a discontinued approach was executed due to late clearance
causing the aircraft to be high on profile. ATC then provided a new approach to runway 07, but it became unstable due to excessive
tailwind conditions, prompting a go-around manoeuvre. The runway was changed to 25, and after assessing fuel and weather, the
crew successfully executed an ILS approach and landed on runway 25.
Flight Safety update: The QAR data indicate during the first approach, the aircraft was positioned above the glide path while
descending toward 2,000 feet QNH at 6NM from the final approach. A discontinued approach was flown at 5NM final at 2,000 feet
AAL. On the second approach, the QAR data indicates that the aircraft encountered a 20 knot tailwind while conducting the ILS
approach, prompting a standard go-around manoeuvre at an altitude of 700 feet AAL.

TAXIWAY INCURSION - SITUATIONAL AWARENESS
During taxi-in on taxiway H, the aircraft inadvertently passed the taxi line leading to gate 28. In the interest of safety, the flight crew
decided to navigate around the adjacent stands using taxiways G, B, D1, and H in order to reach gate 28.
Flight Safety update: Contributing factor mentioned by the flight crew was heavy rain, reducing visibility and increasing the
workload. There was no conflict with other traffic.

August B777 DXB - MAA Approach


2023 LASER BEAMED AT AIRCRAFT
A green laser was directed at the aircraft on approach. The issue was liaised to the ATC.
Flight Safety update: The report has been forwarded to the airport authority who acknowledged the report.

B777 MAA - DXB Parked pre flight


GROUND HANDLING SERVICE STANDARD - GPU FAILURE
The APU was inoperative and the aircraft was being dispatched. A Ground Pneumatic Unit was connected to start the left engine
first but when the Left Engine Start switch was placed to Start, a loud “bang” sound was heard coming from the GPU. After some
time, a new GPU was brought but was also unable to provide the minimum 25 PSI of pressure for engine start. After 50 minutes
from the first attempt, a third GPU was connected and engine start completed uneventfully.
Flight Safety update: The hose and attached connector gave way during the air start due to high pressure. Another pneumatic air
start was sourced from Indigo. Preventative maintenance records were checked and found satisfactory. Maintenance was
conducted on the 12th June 2023 in which the air bleed hose was replaced. Last maintenance was conducted on the 31st July 2023
which states the air hose was tightened .

B777 DXB - MAA Taxi-in


GROUND HANDLING SERVICE STANDARD - WINGTIP CLEARANCE
While taxiing in to bay28, the flight crew noticed that Indigo A320 parked at bay 48 looked a lot closer than the other aircraft parked
next to it, so the flight crew stopped as they were unsure of the wingtip clearance. The flight crew informed ATC and requested
assistance from a wing walker but were unable to reach a contact. After around 10-15 minutes, the airport authority was able to
send a follow me car for assistance.

Flight Safety update: According to the station, the issue on ground frequency was already addressed. GHA discontinued frequency
without intimation to airlines. The new frequency through ATC ground control is 121.9. on wingtip clearance, this was raised with
airports authority earlier and they have advised that the clearance are as per ICAO requirements. They have sent the details which
has been shared with flight operations technical. On markings, Airports Authority has advised that markings are done with
reflecting paints every 15 days. The station are regularly following up on marking and other contribution factors if any with AAI.

B777 MAA - DXB Taxi-out


CONFUSING RUNWAY SIGNS
The flight was taxiing via taxiway B and J to hold short of runway 25. Approaching the underrun to runway 30, the flight crew noted
signage that indicated the underrun was in fact runway 12/30 that is, part of an active runway. They therefore asked for clearance
to cross runway 30. ATC responded that the underrun portion of runway 30 is not part of the runway and therefore does not
require clearance to cross.
Flight Safety update: Airports Authority of India have responded with the following information: It is intimated that the portion of
TWYB to TWYJ is located beyond the intersection of RWYs 07/25 & RWY 12/30. The threshold of RWY30 is located on the other side
of intersection of RWYs. Hence RWY crossing permission from ATC for taxiing this portion when RWY25 in use is not mandated.

B777 MAA - DXB Taxi-out


BURNING SMELL AND SMOKE-OVEN-SOURCE UNIDENTIFIED
On ground whilst the safety video was playing the cabin crew switched on the ovens. Once switched on a burning smell was
emanating from an oven in the mid galley. The cabin crew adopted the firefighting SOPs. Upon further investigation, the source of
the smell could not be identified. MCC were contacted and advised to keep switched off and not to re-set the circuit breakers.
The oven was not used for the remainder of the flight and the smoke and smell dissipated. The captain informed. CAB-21999
Refers.
Flight Safety update: The YC oven was replaced and operation check was satisfactory. The reason for smoke cant be fully
determined until the inspection report is available, however the oven part has been replaced.

B777 DXB - MAA Taxi-in


WINGTIP CLEARANCE CONCERNS
During taxi in via taxiway G with a left turn on taxiway H to stand 29, commented on the tight manoeuvring space in the apron,
concerns with wingtip clearance from an A321 that was parked on stand 46 and presence of ground equipment and vehicles. The
reporter makes suggestions to mitigate the threats during the entry to the apron such as Captains only taxi and publication of a
CoNotam.
Flight Safety update: The report has been forwarded to FOP for a wingtip analyses. The report has been forwarded to the FTP-B for
evaluation of the suggestions proposed by the reporter.

B777 MAA - DXB Pushback


INCORRECT PUSHBACK
Aircraft was cleared to push facing East for runway 25, abeam stand 30, however, was pushed back abeam stand 28R. The parking
brake was set and the pushback tractor moved away. It was identified that stand 28 was not clear for OAL A320 to park. A pushback
tug was called back to position the aircraft in accordance with ATC instructions.
Flight Safety update: While pushing back EK545 (A6-EPU) from bay 28 facing east abeam 30 the mechanic noticed Air India aircraft
was taxing in taxiway G, indigo aircraft was taxing towards bay 28. As a precaution, he stopped EK 545 in just behind bay 28 before
reaching 30. The Manager of Regional Engineering has briefed the Aircraft technician to be more cautious in the future while
following instructions from the captain during push back procedure.

May 2023 B777 MAA - DXB Take-off


BIRDSTRIKE - NO DAMAGE
During the take off roll on runway 25 a medium sized bird (crow) was struck on the upper fuselage above the F/O #1 window just
before rotation. ATC was advised and carried out a runway inspection with nil findings.
Flight Safety update: an engineering check performed and no damage found.

April 2023 B777 DXB - MAA Parked post flight


CABIN MANAGEMENT-INADVERTENT ACTIVATION OF EVAC COMMAND SWITCH
During cruise it was observed that a cabin crew member inadvertently pressed the evacuation command switch instead of the
lights switch at L3. The cabin crew member was debriefed by the captain and the purser.

APU NOT SWITCHED OFF AFTER ENGINE START – PROCEDURAL LAPSE
The APU was not switched off after engine start. The lapse was identified during climb and rectified. A contributing factor
mentioned was an ATC request to expedite taxi.

2022 RADIO ALTIMETER ANOMALY
The aircraft had a previous MEL 34-33-01 Radio Altimeter System which was in active during the concerned sector. During the climb
and while passing 17,000 feet QNH, the Radio Altimeter indicated a count down from 2500 feet to 400 feet. The flight continued to
destination and during approach the flight crew noticed a discrepancy of 60 feet between the 2 radio altimeter indication and with
LAND 3 indicated on the ASA. A techlog entry was made.
Flight Safety update: The report has been forwarded to Engineering and replied that the concerned issue have occurred in the past
as follows. On the 16th of FEB the first report was raised and a System resets were carried out as per standard maintenance
procedure. The system test passed satisfactorily. The fault was reported again on 22th of FEB in RUH airport and the required MEL
was applied by the outstation engineering to limit the operational impact. Once the aircraft was back in DXB, the engineering
followed the troubleshooting manual and carried out appropriate wiring checks. The subsequent tests were carried out and the
system test was passed satisfactorily. Therefore, the MEL had been cleared appropriately. As the fault had reappeared on EK547 on
the 22th of FEB, this led to DXB engineering carrying out further inspections as per the standard maintenance procedures. The
right radio altimeter transceiver was replaced. The system test was carried out and has been found satisfactory.

The repot has been forwarded to FTP-B for review.

November B777 DXB - MAA Approach


2022 MULTIPLE BIRDSTRIKES-NO DAMAGE
On approach passing 1800 feet a bird strike took place. Continued to destination. Engineering found blood stains in the RH Landing
gear and wing with no damage.

FIREWORK DURING APPROACH
During approach to runway 07, the flight crew reported the hazardous of close proximity of the firework activity to the aircraft flight
path which caused audible and visual issues during critical phase of flight. The launch of the firework was noticed to be close the
airport parameters. The ATC was informed and a visual aircraft inspection was carried out post flight to confirm aircraft damage
with nil findings.

Flight Safety update: The QAR indicate that the approach was flown in variable and gusty wind conditions and a go-around was
flown from 790 feet aal. The remaining fuel at touchdown was equal to the FRSV of 2.9 tonnes.

A cabin crew member used an aerosol spray in the toilet causing activation of the smoke alarm. Crew member was debriefed.

GROUND HANDLING SERVICE STANDARD - PASSENGER CABIN SEAT DISCREPANCY
During pushback, the cabin crew identified a discrepancy in a passenger's name and seat number. The aircraft was towed back to
the parking position. Based on the communication with the ground team, it was confirmed that a manual amendment was made
to the boarding passes by the ground staff but was not updated in the system.
Flight Safety update: A Gold SW Member was upgraded from 24C to 7A due to over sales, and another passenger was provided
seat 24C. The boarding card was manually changed to 7A for the Gold Member. All updates were included in the boarding system
at counter closure. The check-in history was reviewed and found correct.

LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING
GPS jamming was experienced on arrival to MAA, after landing, when EICAS ADS-B OUT L and RUNWAY POS displayed momentarily
but cleared immediately. After parking and setting up for the return flight various GPS status messages and associated EICAS
messages would not clear. MEL was applied in coordination with MCC. After departure all GPS related status and EICAS messages
cleared.
Flight Safety update: Although not a usual location for GPS jamming, it has been sporadically reported at MAA previously. The
aircraft showed one item of unusual history at HAN, although this is a sporadic location too, and both the aircraft and the location
will be monitored for further events. External electronic jamming is the most probable cause of the reported GPS signal loss. Since
January 2015 there have been reports of loss of GPS signal, and CoNOTAMs were promulgated for areas subject to such events.
Preventative maintenance actions have been put in place to reset MMRs after GPS jamming events, EKIB-65 issued to 777 flight
crew, followed by the GPS Signal Interference NNC on 09/09/2021, which affect the 777 only. A Safety Issue has been raised for
wider issues surrounding GPS spoofing.

January B777 DXB - AMS Cruise


2024 LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING-SPURIOUS GPWS ACTIVATION
GPS jamming was experienced in the Ankara and Istanbul FIRs. A time shift occurred. Approaching position EVGEG, EGPWS
TERRAIN PULL UP activated for two seconds at FL340. The GPS position showed the location over Crimea.
Flight Safety update: The QAR data have been requested. External electronic jamming is the most probable cause of the reported
GPS signal loss. FCN 2023-050 GPS Signal Interference has been published to provide generic information about GPS signal

December B777 DXB - AMS Cruise


2023 LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING
GPS jamming was experienced in the Tehran, Ankara and Sofia FIRs. Approaching position EVGEG the clock shifted almost five
minutes (from 1544z to 1549z). After 37 minutes, near to position BALBI, the GPS jamming ceased and the clock returned to the
correct time.
Flight Safety update: External electronic jamming is the most probable cause of the reported GPS signal loss. FCN 2023-050 GPS
Signal Interference has been published to provide generic information about GPS signal interference, its effects and Emirates’

B777 DXB - AMS Cruise


LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING-SPURIOUS GPWS ACTIVATION
GPS jamming was experienced in the Sofia FIR and a time shift was evident. EICAS TERRAIN POS displayed. EGPWS TERRAIN PULL
UP PULL UP activated at FL340 and was assessed to be spurious.
Flight Safety update: The QAR data indicate that the GPS position jumped by 193NM over three seconds and five seconds later the
ANP peaked to 20NM. The GPS position moved to be over Sevastopol at a negative GPS altitude. The aircraft rolled to the left to a
maximum of five degrees, as ANP continued to spike to 20NM several times. 36 seconds after the map shift, EGPWS PULL UP
activated for five seconds at FL340, with the aircraft rolling to the left again, to a maximum of seven degrees. Several more spikes of
large ANP and small changes in roll and heading occurred over the next two minutes. Then the GPS altitude recovered to accurate
values and the GPS position drifted back to the actual position over the following 20 minutes. External electronic jamming is the
most probable cause of the reported GPS signal loss. FCN 2023-050 GPS Signal Interference has been published to provide generic

November A380 DXB - AMS Cruise


2023 LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING-SPURIOUS GPWS ACTIVATION
GPS jamming was experienced in the Ankara FIR at position NINVA at FL380. EGPWS TERRAIN AHEAD, PULL UP activated.
Inconsistent terrain information was displayed on ND. The warning was cancelled as it was assessed to be spurious. The following
ECAM messages were displayed: GPWS 1+2 FAULT, TCAS 1+2 FAULT, SURV PRED W/S 1+2 FAULT, PWS 1+2 FAULT, GPS 1+2 FAULT
and GLS 1+2 FAULT.
Flight Safety update: The QAR data indicate that EGPWS TERRAIN, TERRAIN, PULL UP activated at FL380, for 69 seconds. The GPS
position was erroneous, showing the aircraft stationary over Syria at 1,200 feet for the period of the warning. External electronic
jamming is the most probable cause of the reported GPS signal loss. FCN 2023-050 GPS Signal Interference has been published to
provide generic information about GPS signal interference, its effects and Emirates’ experience and countermeasures. For aircraft
related information, refer to the respective type specific documentation. As indicated previously by AIB, in these events, the AESS
considers that GPS altitude to be valid, which leads to a spurious monitoring of the radio altimeters, which it then rejects as invalid,
as they do not match the GPS altitude. This results in the rejection of all three radio altitude readings by the AESS. Finally, as the
TCAS, GPWS and PWS functions of AESS use radio altitude input and these are rejected, then these three functions are lost, as
reported.

B777 DXB - AMS Cruise


LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING-SPURIOUS GPWS ACTIVATION
GPS jamming was experienced in the Ankara and Sofia FIRs. Various unspecified EICAS messages were displayed. RAD NAV INHIBIT
was switched OFF. A COMM message was received stating that a time shift had occurred. The clock was confirmed to be not normal
with approximately five minutes of the correct time. ANP increased to 20NM and then back to a normal value for few seconds on
multiple occasions. EGPWS TERAIN, PULL UP activated at FL340 for two seconds in the Ankara FIR, over the Black Sea and
considered to be spurious. The QRH GPS Signal Interference was referenced and MMRs needed to be reset. Shortly after the GPS
functionality recovered.
Flight Safety update: The QAR data indicate that EGPWS PULL UP activated twice, once for two seconds, and then 39 seconds later
for another two seconds. The GPS position and altitude was erroneous and showed the aircraft stationary over Sevastopol at a
negative GPS altitude. External electronic jamming is the most probable cause of the reported GPS signal loss. FCN 2023-050 GPS

B777 DXB - AMS Cruise


LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING-SPURIOUS GPWS ACTIVATION
GPS jamming was experienced in the Ankara FIR. At FL340 after crossing position NEGEM into the Sofia FIR, a COMM message
displayed regarding a change in aircraft clock time by 5 minutes. A few moments later, EGPWS TERRAIN warning and PULL UP
activated for few seconds. Autopilot and Auto thrust remained engaged and no action was taken. All aircraft systems returned to
normal including the clock after exiting the GPS signal interference area.
Flight Safety update: The QAR data indicate that EGPWS PULL UP activated for five seconds at FL340 over the Black Sea on the
border of the Ankara and Sofia FIRs. The GPS position and altitude was incorrect, showing the aircraft stationary over Sevastopol
with a negative GPS altitude. A time change was not recorded. External electronic jamming is the most probable cause of the
reported GPS signal loss. FCN 2023-050 GPS Signal Interference has been published to provide generic information about GPS

October A380 DXB - AMS Cruise


2023 ATC SERVICE STANDARD - CALLSIGN CONFUSION BY ATC
While in the Baghdad FIR on 123.0 MHz, ATC called the wrong callsign UAE87 six times before calling UAE84N. The controller
apologised after realising the error.
Flight Safety update: The confusion might have been due to UAE87 in close proximity to UAE84N.

B777 DXB - AMS Cruise


EICAS AUTOTHROTTLE DISCONNECT
Approaching top of descent, an EICAS AUTOTHROTTLE DISC activated together with an EICAS THRUST ASYM COMP. At
approximately 5,000 feet QNH, the AUTOTHROTTLE DISC self-cleared and operated normally for the remainder of the flight. A
technical log entry was made.
Flight Safety update: Engineering confirmed that the aircraft lost the thrust asymmetry compensation (TAC) function. Hence, the
autothrottle disconnected. As a precaution, engineering applied MEL 27-02-08. The MEL was cleared after 12 sectors on the
8/10/23 as the fault did not reoccur.

September B777 DXB - AMS Cruise


2023 MEDICAL-CABIN CREW ILLNESS-STOOD DOWN FROM DUTY-GROUND MEDICAL SUPPORT CONTACTED
During cruise a cabin crew member fell ill. Ground Medical Support were contacted and as per their advice medication was
administered. The cabin crew member was stood down from duty.

WEIGHT AND BALANCE
During cruise, new load sheet version was received by acars stating 14pcs/98kg of cargo were found in the ramp after departure.

2023 LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING
GPS jamming was experienced in the Ankara FIR earlier in the flight, which resolved itself. Upon arrival in AMS on the ILS approach
to runway 18C, when established on the localiser at 2,500 feet, EICAS NAV UNABLE RNP displayed and the navigation source
reverted to inertial. The FMS POS revealed that the GPS R position was missing. RAD NAV INHIBIT was selected to OFF.
Flight Safety update:
The PLF shows no active faults at the time of approach and landing, and only inactive faults, from the earlier jamming. The level of
ADS-B coverage around AMS, which was recorded as between 0-2% on the date of the report. The event on approach may have
been a latent effect from the earlier jamming, for which an MMR reset in cruise may have prevented. But it is also possible that
there was some transient jamming from a handheld device that affected the aircraft on approach. Since January 2015 there have
been reports of loss of GPS signal, and CoNOTAMs were promulgated for areas subject to such events. Preventative maintenance
actions have been put in place to reset MMRs after GPS jamming events, and EKIB-65 issued to 777 flight crew, followed by the GPS
Signal Interference NNC on 09/09/2021, which affect the 777 only. A Safety Issue has been raised for wider issues surrounding GPS
spoofing.

A380 DXB - AMS Cruise


ATC SERVICE STANDARD – COMMUNICATION CONFUSION
Reaching position NINVA, ATC (Baghdad 132.875) instructed the flight to contact ANKARA on 127.925. The flight crew acknowledged
this instruction and contacted ANKARA. However, a short while later, they received an ACARS message requesting contact with
Baghdad on frequency 125.9. Despite several attempts, the flight crew could not establish communication with Baghdad on that
frequency. Subsequently, once the aircraft was already in communication with ATC (129.7), they were again instructed to contact
ANKARA on 127.925, the same frequency they were already in communication with.

B777 DXB - AMS Cruise


DISRUPTIVE PASSENGER-RESTRAINT
During cruise a passenger moved from their seat in YC and sat in FC and refused to return to their original seat. It was confirmed
by the passenger's companion that the passenger had autism. Then the passenger went to the FC galley and tried to access the
cockpit by pushing the door and pressing the keypad. The captain was informed. A formal warning was issued and a decision was
made to restrain the passenger. Security services met the aircraft on arrival.

LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING
GPS jamming was experienced in the Ankara FIR and both GPS did not return to functionality until after engine shutdown.
Flight Safety update: Non-recovery of GPS on the A380 occurs in less than 1% of GPS jamming events recorded in the technical log.
External electronic jamming is the most probable cause of the reported GPS signal loss. Since January 2015 there have been
reports of loss of GPS signal, and CoNOTAMs were promulgated for areas subject to such events. Preventative maintenance actions
have been put in place to reset MMRs after GPS jamming events. A Safety Issue has been raised for wider issues surrounding GPS
spoofing.

LOSS OF GPS-SUSPECTED ELECTRONIC JAMMING-SPURIOUS GPWS ACTIVATION
GPS jamming was experienced in cruise. EGPWS PULL UP TERRAIN occurred at FL340. GPS R did not recover, and, in accordance
with MCC, the GPS SIGNAL INTERFERENCE NNC was completed with MMR reset.
Flight Safety update: The QAR and PLF data showed that the aircraft experienced at least two areas of GPS jamming, with the first
being over BGW airport at approximately 0230 UTC, and then the second being near to position SIVAS in the Ankara FIR. During the
later event, an EGPWS PULL UP warning displayed at FL340 for a period of six seconds at 0325 UTC. GPS R did not recover, and
when in the Budapest FIR at 0517UTC, the flight crew completed the GPS SIGNAL INTERFERENCE NNC, which resulted in MMR R
being reset in coordination with MCC. External electronic jamming is the most probable cause of the reported GPS signal loss. Since
January 2015 there have been reports of loss of GPS signal, and CoNOTAMs were promulgated for areas subject to such events.

2022 MEDICAL-PASSENGER ILLNESS-DIVERSION
During cruise a passenger became ill. In consultation with Ground Medical Support and VPNC the flight diverted. The passenger
was met by medical services on arrival.
