Instructions to Donor and Borrower(s):

Donor and Borrower(s) must provide a copy of (i) Donor’s check or other evidence of transfer of the Gift,
and (ii) Borrower(s)’ bank statement or other evidence of receipt of the Gift.

Under applicable guidelines, a gift may typically be used for a principal residence or second home, but
not on investment property. Gifts may typically be used for down payment, closing costs, or financial
reserves subject to minimum borrower contribution requirements (provided that equity gifts may not be
