Using the template below, create an FLRA for one chosen trade:
‐ Bricklayer
‐ Carpenter
‐ Concrete Finisher
‐ Electrician
‐ Industrial Mechanic
‐ Ironworker (rebar)
‐ Plumber
‐ Roofer
‐ Welder
Task Hazards Control Risk Level
Each FLRA should include:
1) 8 Tasks
2) 6 Hazards for each task
3) Control measure for each hazard
4) One Risk Level rating for each task

Use the following matrix to determine risk level:

High Risk ‐ Immediate risk control required. Apply Standards, Safe operating procedures,

Practices, Job hazard assessments and Field level Risk assessment processes

Medium Risk ‐ Risk Controls must be considered. Appropriate risk controls shall be applied for
M repetitive occurrences. Apply standards, safe operating practices, Job hazard assessments field
level risk assessment processes

Low Risk ‐ Risk reduction controls may be required. Apply safe operating practices and field

level hazard assessment processes.
