How to create a Application Form

In order to create ana application form, you need to create a variety of skills.
When you are creating application forms us A4 paper unless otherwise instructed.
You start of by typing as much as the form you can normally then you use various
techniques to add additional elements.

Drawing Lines
Tab Stops
You typically draw horizontal lines using underscore auto shape and tab stops.
Using underscore the easiest way to draw a line is to hold down the shift key and
the underscore key until the line is the length that you want

AutoShapes
The advantage of this approach is that you can use it to draw a line on any part of a
page using tab stops with leaders. The third approach is the one you would have
used to create programs and table of contents using tab stops. This is usually the
best method to use in the following situations.
1. If the line extends to the end of the page
2. If you need multiple lines in the same row
3. If you have multiple lines in the same length aligned vertically although in
some cases it is better to use a table

Drawing Boxes
There are two main ways to draw boxes there are auto shapes and to use tables
Using Text boxes
The easiest way to draw a box that the user is supposed to tick is to used an empty
text box

Using Tables
If you have several boxes which are aligned side by side, it is better to use a table.

Using Text Boxes to Precisely Position Texts

If you remove the borders and fill the colors from the text boxes you can use them
to precisely position items of text on the form

Assignment
Create an application form for the young peoples sports club the form should
contain the following instructions.
 The person’s name and address
 The telephone numbers.
 What sports they play (provide boxes for the following sports;
Football,Cricket,Basketball,Tennis,Netball,Hockey
 Who to contact in case of an emergency.
 Must have space to enlist any medical conditions they suffer from
 Be sure to create a letterhead.
