The National Training Program -CWTS 2, has requested this waiver in connection with its
Community Immersion and Project Implementation Offered this 2 nd Semester of AY 2022-2023
in which is accepted and confirmed. The student implementor,

Together with his/her parents or guardians acknowledge that permission granted to him/her is
made subject to condition which he/she hereby accepts and agrees to, that university will not
assume any responsibility what soever for any injury or accident due to their own negligence
which may happen while undergoing Community Immersion and Project Implementation.

This waiver will take effect from the duration of this program. Feb 13, 2023- June 17, 2023

PLP students who are currently enrolled in the NSTP-CWTS must undergo community exposure
and participate actively in any community –based activities as one of the major requirements
for passing the subject.
