FLOW OF ENROLLMENT

Step 1: Brigada Eskwela

To help us prepare our school for the opening of classes, you are invited to render an
hour voluntary community service. Secure your Brigada Eskwela Slip and Enrollment
Procedure Checklist from the following focal persons in-charge per grade level:

*Incoming grade 7, Balik- Aral and Transferee proceed to Step 2. For incoming Grade 8, 9 and 10
learners shall proceed to Step 3 for BMI Screening Assessment and Step 5 for Final Enrollment.

Step 2. Secure Enrollment/Cumulative Form

For incoming Grade 7,8,9,10 Balik- Aral and Transferee. Secure your Enrollment Form
from the Grade Level Enrollment Committee.

Step 3: BMI Screening Assessment

For incoming Grade 7,8,9,10 Balik- Aral and Transferee. Proceed to Grade 10
classroom.

Step 4.Take the following examinations

Incoming Grade 7 only

A. Numeracy Assessment

B. Reading Assessment

Step 5: Enrollment

For incoming Grade 7 and Transferee learners, bring the following requirements:

1. PSA Birth Certificate

2. Report Card

3. 1 (2x2) id picture

4. Transferee should be accompanied by their parents/guardian

* For continuing grade 8,9 and 10. Please proceed to grade 10 room for releasing of
cards, then you may see your respective year level Enrollment Committee for enrolment.
