Executive Summary

The management of M/s Parasakti Cement Industries Limited, Jettipalem had


permission to conduct Safety audit by Safety Department. Audit for their plant located
at Jettipalem vide work Document Pcil Safety. Reference no: PCIL/SAF/AUDIT on 20th
January, 2020.

Accordingly, following Auditors from Safety Department visited the plant and carried
out safety audit
from 27th January to 29th January, 2020.

The safety audit was carried out in line with IS 14489:1998 (Reaffirmed 2002), Code of
practice on Occupational Health and Safety Audit.

The Safety Audit is a systematic, objective and documented evaluation of the


Occupational Safety and Health systems and procedures to identify the areas of
improvement. The evaluation process involves site visit, Discussion with the audit
team members and verification of documents and records pertaining to OHS.

Some of the salient findings of the study are presented below:

Positive Observations:

 Management initiatives towards improvement of safety – Well designed fire


protection system for WHRP.
 Fire Hydrant Water suppression system for coal mill, bag filter and fine coal bin
of Line 2
 Provision of explosion vents on Coal bag filter
 Housekeeping is maintained Excellent each and every corner in plant.
 Speed limit restriction for vehicles in the plant (15 kmph) and breathalyzer
checks

Areas of Improvements identified in the audit are given below:

Safety Management System

1. Structural stability tests not conducted for buildings and other civil structures
(Silos/ Hoppers)
2. No Management of Change (MOC) system was found to be followed in the plant.
3. HIRA shall be established for all activities and task to ensure adequate control
measures are implemented to reduce the risk to the acceptable level.
4. Occupational health risk mapping shall be done. Employees exposed to various
occupational health risks shall undergo medical examination to ascertain the
health status.

Safety Operations & Maintenance

5. Fire Resistant belts as per standards were not provided for the coal conveyors.
6. Automatic sprinklers were not installed between outgoing and return belts of
each conveyor. Heat detection system was not present.
7. No emergency plans were developed for coal tunnel fire.
8. Man basket used was also not marked with SWL.

1.0 Introduction

The management of M/s Parasakti Cement Industries Limited, Jettipalem had


permission to conduct Safety audit by Safety Department. Audit for their plant located
at Jettipalem vide work Document Pcil Safety. Reference no: PCIL/SAF/AUDIT on 20 th
January, 2020.

Accordingly, following Auditors from Safety Department visited the plant and carried
out safety audit
from 27th January to 29th January, 2020.

The safety audit was carried out in line with IS 14489:1998 (Reaffirmed 2002), Code of

The Safety Audit is a systematic, objective and documented evaluation of the

1.1 About Parasakti Cement Industries Limited. Jettipalem

Parasakti Cement Industries Limited (PCIL) has two cement manufacturing units with
capacities of 1700 Metric Tonnes /day and 2600 Metric Tonnes / day The second
Waste Heat Recovery Power plant powered with the latest and modern technology and
infrastructure has started operations in 2019 and has poised the company on an
energized growth path to achieve new performance levels and service standards. 145
on-roll employees are employed. The plant is certified for IMS (9001) and located near
Jettipalem village in Rentachinthala Mandal, Guntur district, Andhra Pradesh state.

The nearest land marks are Nagarjuna Sagar Tail Pond Project constructed over
Krishna River at an approximate distance of 0.5 kilometers to South west corner. The
second land mark is Satrasala Shiva temple on the banks of Krishna River at an
approximate distance of 0.75 kilometers to North west corner.

The plants water requirement is met by deep well bores located in premises. The power
supply to the plant is met by 132 KV transmission line from 220 KV substation
located at Pasarlapadu village approximately at a distance of 5 kilometers. The plant is
connected by pucca road from Macherla and Dachepalli state highway at a distance of
9 KM. The nearest railway station is at Rentachinthala at a distance of 15 kilometers
and major railway junction is at Nadikode on Guntur – Hyderabad line.

The Cement Manufacturing process at PCIL works is briefly explained. In all below
mentioned process Quality Control Assurance implemented at Statutory and
regulatory aspects and complied every stage.

1.2 Safety Department:

The Safety Department which was fulfilling the safety standards and improving safety
culture by giving awareness TBT (Tool Box Talks)/ PEP Talks and internal & external
safety trainings conducted each and every department. Regularly monitoring and
discussing the safety issues and Permit to work and strict implementation of LOTO.
The Safety Committee Meetings conducted once in two months with effective
participation of workmen & management representatives. The Motivational Programs
Carried out for the year calendars like Road safety Week, National Safety Day, Fire
Service Week, Electrical Safety Week.

To follow the safety rules /guidelines which were made by the management and take
proper training on safety. All Workers are following safety precautions in all aspects at
hazardous job areas of working at heights and strict implementation of work permits
and Line Clearance (LC) from concern department and belt conveyor check list,
electrical safety Fire extinguishers & Fire Hydrant check list & safety sign boards and
also taking necessary safety precautions for safe work environment.

The worker failure to maintain safety the entire family will suffer to that mistake.
While working we don’t get any tensions and work safely to avoid accidents. With
regard to, all the workers must wear the safety equipments from the house like as
safety shoes and safety helmet. Housekeeping is a very important role in maintaining
safety. Every concerned must contribute to housekeeping. Safety is a continuous
process, not for a day or minute, it is for entire our life and it’s a team work.

The management is emphasizing as first safety and next all. With the continued
support of the Management we are able to achieve “ZERO ACCIDENTS” from last 12
years.

For this achievement, we are grateful to our young and talented staff and workers
whose work and labour have enabled industry to take a big stride towards progress.

We have to improve ourselves and learn new technolgies on safety day to day life. The
world changing on safety day by day with advanced technologies and should follow
and protect from our working activities.

1.3 Objectives of the Study

1. To carry out a systematic, critical appraisal of all potential hazards involving


personnel, plant, Services and operation method.
2. To ensure that Occupational Safety and Health system fully satisfy the legal
requirements and those of the company’s written safety policies, objectives and
progress.
3. Suggest Good Engineering practices followed in similar facilities

1.5 Methodology

The audit plan was set up with inter related departments with co-coordinating to
collect the details on the operations, existing systems and various records connected
with Safety Management System.
The Safety Department had prepared a checklist for applicable statutes and standards
for use during after the site visit.

Rules 1981;Petroleum Rules; MSIHC Rules; MV Act / CMVR Rules
Other applicable safety standards (National & International)
Good engineering practices followed in similar industries.

1.7 Report writing

The observations made during site study at various sections and respective
recommendations have been categorized into broad sub-headings like Safety
Management System, Safety in Operations and Maintenance, Fire Prevention and
Protection, Electrical Safety, Emergency Management and Hazardous
material/chemicals. The report also captures applicable standards / statutes against
which each observations / recommendations have been benchmarked.

Annexure-11:
PCIL/PP/WI 08:- Standard operating procedure –procedure for refractory lining
in KILN I&II
1. Before taking shutdown check the following tools for kiln lining.
2. Tier wedges, chisels, wooden runners, kiln jack, jack rods.
3. Coating pipes, lighting arrangements, wooden flanges.
4. Coconut ropes, wedge plates, air breaker, brick cutting blades
5. Water supply, compressor air, rubber belt, gamelans, brooms,
6. Mortar, key bricks, kiln bricks etc
7. Inform to masons and man power
8. Check the kiln shell temperature .it should be ambient temperature.
9. Take shutdown permissions (L.C.) from all servicing departments.
10. Open kiln hood doors fully.
11. Remove the burner to outside with help of servicing departments.
12. When kiln inside is high temperature, start the bag fan and maintained required
draft.
13. If kiln is not empty, release the L.C. and start the Auxiliary rotation for some time.
14. After cooling the kiln stop the kiln auxiliary and Take L.C.
15. If required put the kiln tier wedges.
16. Take coating area measurement of kiln inside.
17. After measurement starts the coating remove work.
18. After removing of coating start kiln auxiliary drive for kiln emptying.
19. When kiln is empty, stop the auxiliary drive and check the kiln lining thickness.
20. After conformation, start the kiln lining work.
21. After removing, start the cleaning with man power.
22. After completion of shell cleaning, inform to Mechanical section in charge to fix
the paint.
23. After mech. Clearance starts the kiln lining.
24. Any end joint brick should be more than half of the length.
25. After completion of half round of lining fix the Jacks.
26. Remove all wastage in kiln inside and carefully rotate the kiln auxiliary.
27. After completion of ¾ lining again fix the jacks and rotate kiln.
28. After completion of circle rotate kiln for key position.
29. Check the key brick tightening.
30. After completion of key work remove the Jacks and clean the material.
