    The master of the Ocean Lynx is requesting an enclosed space entry permit to inspect tanks on October 6, 2016. Atmospheric testing found oxygen levels of 21% and LEL below 1% in tanks 2W, 4W, 6W, and 7W. All tanks have been gas freed according to company and ISGOT procedures. A safety meeting was held with crew entering the tanks. The portable gas meter was last calibrated on October 3, 2015 and tested before use on October 6, 2016. A risk assessment file is attached.

    Please kindly requested for Enclosed Space Permit to purpose Tank’s Inspection on 06 Oct
    2016.

    a. Vessel Tank’s (WBT 2W, 4W, 6W, 7W) has dry and checked LEL and follow as per
    Company SMS Procedures and ISGOT guidance ( Atmosphere 21 % and LEL < 1 % of
    Tank’s.

    b. All Tank has been Gas Free

    c. Safety Meeting conducted by Master, C/O, Duty Officer, Bosun and the crew who enter.

    d. Portable Gas Meter last Calibration : 03 Oct 2015

    e. Portable Gas meter tested before use : 06 Oct 2016

    Attached herewith Risk Assessment in the File

    Please issue Enclose Space Entry Permit as Necessary.
