AFFIDAVIT OF LOSS

I, Mr./Mrs./Ms. REO JOHN E. ANGET, of legal age,

single/married/widower/separated, Filipino, and a resident of Tagum City, after
having been sworn to in accordance with law, hereby depose and state:

1. I am a holder of Professional Driver’s License duly issued by the Land

Transportation Office (LTO) with license no. L02-11-006389;
2. On March 19, 2024, I searched for my wallet but I noticed that it was
no longer inside my wallet where I usually put it;
3. I immediately exerted efforts to look for it but my efforts proved
futile since it was nowhere to be found, and may now be considered as
LOST/MISPLACED;
4. That the wallet was not confiscated by any Government authorities in
violation of RA 4136 or any other related laws;
5. That I am executing this Affidavit to attest to the veracity of my
statements and as support in my request for the issuance of a new driver’s
license to be issued by LTO-Panabo City; and
6. I am executing this Affidavit to attest to the truth of the foregoing
facts and for whatever legal purpose it may serve the premises..
