AFFIDAVIT OF EXISTENCE OF RIGHT OF WAY

_____________________________, after having been duly sworn to in accordance with law do
hereby deposes and states, that:

1. I am an owner of a certain parcel of land located at ________________________,

with an area of FOUR THOUSAND and FIVE (4,005) SQUARE METERS, more
or less, covered and embraced by TRANSFER CERTIFICATE OF TITLE (TCT)
No. ______________________;

2. Portions of the said parcel of land are Lot 2-A-1, Psd-1-012769 and Lot 2-A-2-G
which is located on the northern part, and Lot 2-A-2-A and portion of the adjoining
lot on the north-west part;

3. Lot 2-A-1, Psd-1-012769 and Lot 2-A-2-G, owned by one and same individual, has
an existing road located on the north-east part which is the main road;

4. The other lot, Lot 2-A-2-A and the portion of the adjoining lot, it also has an
existing road and water way that is the continuation of Lot 2-A-2-J (Road and
Waterway) from the southern portion that passes through the western portion going to
Lot 2-A-2-A and near/adjacent to the said portion of the adjoining lot;

5. I am executing this affidavit to attest to the truth of my foregoing statements and for
all legal intents and purposes it may serve.
