Subject: Request for Borrowing Municipal Services for Student Field Trip

Dear, Mayor Santos,

I hope this letter finds you in good health and high spirits. As an esteemed member of our esteemed
municipality, I am writing to formally request your esteemed assistance in facilitating a memorable and
educational field trip for the students of Western Philippines University(Quezon Campus).

Our educational institution, Western Philippines University, has always strived to provide a holistic
learning experience for our students. In line with this commitment, we have organized a field trip to Iwahig
Prison and Penal Farm on November 10, 2023, accompanied by 47 students and 5 teachers.

Understanding the importance of such experiential learning opportunities for our students, we humbly
seek your support by allowing us to borrow certain municipal services for the smooth and safe execution
of this educational endeavor. Specifically, we kindly request the following services:

1. Transportation: We kindly request the provision of 5 buses/vehicles to transport our students and
teachers to and from Western Philippines University (Quezon Campus). This will ensure their safety and
allow for a more organized and efficient trip.

2. Security Assistance: We kindly request the provision of 5 security personnel to accompany our
students and teachers throughout the trip. Their presence will ensure the safety and well-being of
everyone involved, providing reassurance to both the school administration and the parents.

3. First Aid: Considering the unpredictable nature of field trips, we kindly request the on-site presence of a
certified medical professional or access to a first-aid station to address any unforeseen medical
emergencies that may arise during the trip.
Please be assured that we will take complete responsibility for the proper usage and upkeep of the
borrowed municipal services. We will ensure that the vehicles are returned in the same condition they
were borrowed, and that all necessary precautions are taken to ensure the safety and security of both the
students and the municipal resources.

We kindly request a response to our request at your earliest convenience, as we need to provide our
students and teachers with the necessary details and arrangements for the field trip. Your support in
facilitating this enriching experience for our students would be highly appreciated.
