1962 - Polio Dextroscoliosis is the abnormal curvature of the spine

Ang Polio, or poliomyelitis, is a crippling and potentially
fatal disease caused by the poliovirus. Sa Pilipinas, ang
mga bata below 1 yr old ay binibigyan ng three doses of
polio drops and one dose of inactivated polio vaccine.
However, The patient stated na hindi sya nabakunahan
against polio, Tapos nagkaoutbreak… noong sya’y two
and a half years old, causing her to contract the
condition.
1964 –1967 Asthma
The patient was diagnosed with asthma noong
sya’y four years old. But the symptoms subsided before
she turned seven. The triggering factor was unknown
although sabi nya SHE experienced difficulty of
breathing, drowsiness, confusion, and wheezing She
was using a prescription inhaler to relieve her problems.
1972 Goiter
The patient was diagnosed with Goiter when she was 12
years old. Iodine deficiency was the etiology of the
condition. Radioactive Therapy po yung naging treatmet
nya. The therapy reduces or blocks hormone production
and reduce goiter growth. Fortunately, now her goiter
is treated and does not prompt any further
inconveniences.
Previous Medical Illness:
Scoliosis
At the age of 23, napansin ni patient na uneven
po yung shoulders nya. Noong sya’y nagpa-xray it was
found that her spine was deviated to the right side.
doctor recommended scoliosis bracing. There were no
additional treatments reported.
Diabetes secondary to chemotherapy
Habang nagchechemo si patient tumataaas daw po
yung bp at blood sugar nya. Then later on the doctor
diagnosed her with Type 2 diabetes and prescribed an
insulin pen to be self-administered.
Vaccination/ Immunizations
Except for the polio vaccination, the patient was able to
get all childhood immunizations as well as cholera
vaccine. Fully immunized na po si patient against covid-
19 through Sinovac vaccine. On September 25, she
received her first Pfizer booster. After receiving the
booster shot, the patient claimed that her chest
discomfort had subsided.

The main symptoms of goiter include:
 A lump in the front of your neck, just below

caused by diet, stress, inflammation or even
