RELEASE AND QUIT CLAIM

services therewith and/or the termination thereof.

In consideration of the said payment, I have remised, released and forever discharge and by these,
do for myself, my heirs, my successors and assigns, remise, release and forever discharge PSMPC and it’s
directors, manager’s, agents representatives, successor’s and assigns, from any and all manner of actions,
cause or causes of action, suits claims and/or demand incident to or by reason of arising out of having
rendered services with __________________________. That I have not authorized and will not
authorized and entity firm or person, to file a case in my behalf before any court of law, the Department of
Labor and employment or any of the government agency regarding any matter, incident arising out of, or in
any connection with my having rendered services and/or termination thereof.

I hereby declare that I have and fully understood this document this document before signing the
same and that the release and quitclaim hereby given me is made willingly and voluntarily and with
knowledge of my rights under the law.
