DEPLOYED NHWSS PERSONNEL AND ALL OTHERS
CONCERNED

SUBJECT :Reappointment and Other Activities for Approved Personnel under


the 2024 National Health Workforce Support System

The Department of Health (DOH), through the NHWSS, renews its commitment to
pursue an equitable distribution of competent and compassionate health workforce to
improve access to quality health care services in priority areas of need. Pursuant to The
Administrative Order No. 2020-0038 entitled “Guidelines on the Deployment of Human
Resources for Health under the National Health Workforce Support Svstem”, and DO
2022-0451, its operational guidelines, the implementation of the National Health Workforce
Support System (NHWSS) augments Human Resources for Health (HRH) in areas of need
and prioritizes Geographically Isolated and Disadvantaged Areas (GIDAs), low-income class
municipalities, municipalities with indigenous people (IP), national priority areas for
poverty-reduction and peace-building efforts.
Further, the recruitment, selection, and placement process of all HRH as enumerated
in the above-stated pohcies, shall be subject to Civil Service Commission (CSC) Rules on
Appointments and Other Human Resource Actions. Along this line, this memorandum
provides guidelines on the processing of reappointment and other Human Resource activities
under the 2024 National Health Workforce Support System.
I. Submission of Reappointment Documents
All personnel approved for reappointment in 2024 shall submit the following
pre-employment requirements to their respective Provincial DOH Offices no later
than January 10,2024, Wednesday:
1. Three (3) duly accomplished, originally signed and notarized CS Form No.
212, Revised 2017 (Personal Data Sheet and Work Experience Sheet). Notary
date must be no later than January 01,2024.
2. Three (3) photocopies of active PRC ID/License
3. One (1) Medical Certificate (CS Form 211, Revised 2018) originally signed by
a Government Physician with the original results of the following laboratory
examinations:
a. Complete Blood Count;
b. Urinalysis;
c. Chest X-Ray;
d. Drug Test; and,
e. Pregnancy Test (if applicable)
4. Two (2) notarized copies of Statement of Assets, Liabilities, and Net Worth
(SALN) as of December 31, 2023.

One (1) PSA Issued or LCR authenticated copy of Certificate of Marriage (for
newly married female applicants who intend to use their married name if PRC
ID still needs to be updated.
6. Two (2) Photocopies of Training Certificates reflected in PDS.

II. Central Visayas Registry of NHWSS Human Resources for Health

The Central Visayas Registry of NHWSS Human Resources for Health serves
to create a database of relevant personal and work-related information of all NHWSS
personnel for Human Resource Management and Program-related actions.

For inclusion to the registry, all appointed/re-appointed/re-hired personnel


under the composite programs of the NHWSS in Central Visayas shall register/update
their personal information at this link: tinvnrl.com/CHP7NHWSSRegistrv.
Deadline of registration is on January 05,2024.

HI. Official Stations of Barangay-based HRH Personnel

The concerned DOH Representative, in coordination with the City/Municipal


Health Officer, shall determine the barangay assignments of nurses and midwives,
subject to identified priority areas. To ensure adherence to the staffing standards, the
following conditions shall be strictly observed;

HRH Cadre Area of Assignment


a. Nurse II (NDP) Maximum of three barangays with a combined
population not exceeding the standard 1:10,000 ratio.
b. Midwife II Maximum of three barangays with a combined
(RHMPP) population not exceeding the standard 1:5,000 ratio.

Midwives may be rotated in the Birthing Stations/Lying-In Clinics at most


twice a week provided that there is an insufficient mraiber of LGU-hired midwives to
man the said clinical area. All other cadres are prohibited from rotating in the birthing
station/lying-in clinics.

IV. Submission of Performance Planning and Commitment Form

All 2024 NHWSS personnel shall conduct a Performance Planning and


Commitment Activity (target setting) with their DOH Representatives for the
January to Jime 2024 rating period. As an output, they shall submit the SPMS Form 4
or the Individual Performance Commitment and Review (IPCR) Form reflecting
targets for each identified Success Indicator, together with the metadata. The SPMS
form shall be duly signed by the DOH Representative and the personnel.

Considering that the 2024 Strategic Performance Indicators have not yet been
communicated, they shall follow they shall plan for all three (3) Indicators reflected in

their 2023 2nd Semester IPCR, except on the Registration for Provisional PCW
Certfication among physicians, nurses and midwives. Additional Success Indicators
(especially for program point persons) may be added as per agreement between the
DOH Rep. and deployed HRH personnel. FHSIS success indicators (targets) shall
based on the accomplishment covering July 1 to December 31, 2023.

A sample of Performance Planning and Commitment is attached herein for


reference. The accomplished forms and metadata shall be submitted online on or
before January 31, 2024. The link for submission shall be communicated at a later
date.

V. Non-renewal/Separation Procedures

The 2024 Approved NHWSS Personnel who do not intend to renew their
appointment shall signify to the HHRDU through a letter of intent (LOI). Email
submission of an advance copy is highly encouraged. Non-renewing personnel
shall also accomplish all separation documents listed in Annex A and submit them
to the HHRDU Office or to their respective Provincial DOH Offices.

VI. Submission of January 2024 Daily Time Records

Specific for January 2024 only, all 2024 NHWSS personnel shall submit two
separate Daily Time Records (DTRs) for services rendered in January 1-15 and
January 16-31, 2024. Subsequent DTR submission shall remain on the regular
monthly schedule.
