Covid Programme:
Robert Mugabe Clinic

 Screening of patients for Covid by swabbing, oropharyngeal and throat swabbing.


 All nurses and doctors wearing Protective clothing.
 Patients were encouraged to sit one a meter apart wearing masks.
 Hand sanitizing with 70% alcohol was used.
 Nurses taking vital signs, temperature, pulse, blood pressure and oxygen saturation.
 Patients with oxygen saturation of 40-88% where put on oxygen and monitored.
 Prescribed drugs where given to patients by nurses.
 Covid positive patients were sent for isolation places, which were provided by the Ministry of
Health.
 Sick patients were treated asymptomatically, sent home and advised to isolate at home until
Covid results were out.

Medical Wards:

 Hand over, takeover of patients reports done on every shift.


 Daily bed baths to bed ridden patients and pressure area care to prevent bedsores.
 Provision of total nursing care to all admitted patients i.e. 2 hourly turns, bed baths, up baths,
4 hourly oral care, 4 hourly eye care, 6 hourly catheter care, 4 hourly vital signs.
 Infection control activities – damp dusting and cleaning of ward.
 Giving of medication as ordered by doctor.
 Pre and post-operative management of patients going to theater.
 Doing doctors rounds and carrying out orders.
 Assisting doctors during procedure, lumbar puncture, application of POP, insertion of chest
drains.
 Ordering of drugs to maintain ward stock.
 Handling of DDA drug keys and making sure the drugs are kept in a lockable cupboard.
 Pethidine and morphine are kept in a DDA cupboard.
 Feeding of very ill patients during meal times.
 Taking and writing comprehensive reports on patients condition.
 Vital signs such as temperature, blood pressure and respirations done 4 hourly to monitor
patients condition.
 On discharge, a discharge summary is written on the patients card for example, procedure
done and investigations.
 Giving of patients review dates.

Surgical Wards:
Specific Procedures done in surgical wards:

 Dressing of wounds daily.


 Dressing done after damp dusting of cleaning of the ward to prevent cross infection.
 Throat swabs are done to nurses when they suffer throat infections.
 Nurses not allowed to do dressing until throat infection is healed.
 Preparation of patients going to theatre, consent form to be signed and the doctor fully
explains the procedure to the patient prior theatre.
 Blood tests like haemoglobin, group and cross-match are done in case patient bleeds in
theatre and blood will be kept ready in case of emergency.
 2 nurses should take the patients to theatre after pre-medication has been given as
prescribed.
 A detailed report is given to the theatre nurse taking note of any abnormalities.
 From theatre port-operative observations are done, like temperature, blood pressure, pulse
and respirations to monitor patients condition.
 Operation site is observed for bleeding.
 If bleeding occurs doctors must be informed.
 Patient is nursed in a recovery position until recovery so that patient doesn’t aspirate.
 Plaster of Paris is applied to patients with fractures to promote union.
 Circulation checks are done 4 hourly to check that POP (Plaster of Paris) is not tight.
 Patients with burns are nursed in isolated wards to protect them from cross infection.
 Patients who need physiotherapy are taken for physio daily to promote function of affected
limbs.

Outpatient Departments:

 Delegation of duties and supervision of subordinates.


 Screening and treatment of patients as well as referring to the doctor if need be.
 Ordering of drugs and other sundries.
 Reporting all notifiable diseases to infection control nurse.
 Keep records and graphs of disease surveillance.
 Giving of immunization to under 5’s.
 Giving of daily injections to outpatients.
 Doing daily dressing.
 Attending meetings and giving feedback.
 We had three shifts in the department.
 Health education to patients was given.

Pediatrics Ward:

 Daily baths to children in the mornings.


 Feeding of children according to body weight.
 Daily dressings.
 Daily weights are done to monitor child’s growth.
 Admission of breast feeding mothers to promote bonding of mother and child.
 Giving of special diets to malnourished children.
 Drugs are given per kg/body/weight

Isolation Ward:

 Observing, assessing and reporting information on patients condition.


 Diagnosing and referring clients accordingly.
 Initiating appropriate treatment and carrying out of orders accordingly.
 Infection control e.g. prevention of cross infection.
 Health education to clients and relatives.

Gynaecology Department:
We admitted conditions like:

 We admitted cases of abortions e.g. incomplete abortion, threatening abortion, inevitable


abortion and habitual abortions.
 Patients with uterine fibroids admitted and taken for hysterectomy.
 Pre and post-operative procedures were done.
 4 hourly vital signs temperature, blood pressure, pulse done to assess patients condition.
 Investigations like haemoglobin, group and cross match done, urea and electrolytes done
since patients come bleeding.
 Blood transfusion is given to patients with low haemoglobin.
 Procedures like evacuation of retained products is done and manual vacuum extraction done
to pregnancies which are 12 weeks and below.
 Patients with cancer of the cervix, cancer of endometrium, cancer of ovary are also admitted
in this ward.
 Patients with advanced stage of cancer are taken for radiotherapy and chemotherapy.
 Patients with advanced cancer post radiotherapy or chemotherapy are counselled by the
oncology doctor and prepared for home based care

Public Health:

 I worked at clinics.
 Consulting and screening of patients.
 Compiling of statistics e.g. T’s
 Giving of immunizations to under 5’s.
 Ordering and storage of drugs and vaccines.
 Coordinating with the community and community leaders.
 Domiciliary visits e.g. home based care.
 Health education given to patients to be aware about diseases and hygiene.
 Attending meetings and workshop to keep abreast with the new developments.
 Assisting subordinates with filling of leave forms.
 Deliveries done when patients report to the clinic fully dilated.
 Those who report early in labour are referred to hospital where there are midwives for further
management.
