SECTION 02: OBJECTIVE

 This is confirmation of commitment of Black Arrow safe guards protect lives, and property
of Saudi Aramco, and any other third party in connection with the work to be performed.

 To achieve this objective Black Arrow management, in coordination with the concerned
Saudi Aramco Project Engineer Environmental Protection Dept., Loss Prevention
Department, and other applicable agencies as required, are to make joint site visit to
identify the potential hazards of the work to be taken and to initiate control measures
adequate safety for duration of work.

BLACK ARROW HSE objectives are to;

 Ensure that HSE goals are the prime consideration in the Project Execution Plan.

 Achieve a target of ZERO fatalities and ZERO lost time injuries.

 Minimize any impact on the natural environment.

 Implement a safety management system with clear HSE objectives.

 Communicate all the foregoing to the labor force on the project.

 BLACK ARROW overall safety objective is to proactively limit accidents and injuries to
workers and loss of materials and property during all phases of the project and especially
during the construction phase where the risk of injury and losses are greatest.

 BLACK ARROW shall provide a written Construction Safety Plan to JHAH/Saudi Aramco to
cover all aspect of its site activity. This plan shall be submitted for review by JHAH/Saudi
Aramco prior to mobilization to work site.

SECTION 03: Brief Scope of Work

 This project is to Upgradation of HVAC System at Dental Lab Bldg # 54 Dhahran with their
associated duct works, electrical works. Install new (1) ONE FRESH AIR HANDLING UNIT,
mounted on top of concrete pad with vibration isolator pads, properly leveled and required
anchor bolts. Install new thermostat complete with wires and conduits. All supplied thermostats
should be programmable with DDC PANEL. Insulate new duct works with 2” thick fiber glass
insulation blanket with aluminum vapor barrier. The completed air duct works shall be identified
and marked supply Air/Return Air with air flow arrow directions as per JHAH/Saudi Aramco
painting and color code standard.

ELECTRICAL Demolition Works:

 CONTRACTOR shall disconnect/remove controls and power supply wiring of exhaust fan, electric
duct heaters and motorized volume dampers serving the dental lab area as shown on project
drawing. Remove control wiring and conduit back to source or control panel
 CONTRACTOR shall remove and salvage all existing electric duct heaters, and associated
accessories serving that are affected by the new work at dental lab area as shown on the
project drawing and without affecting the other spaces

New Work:

 CONTRACTOR shall supply and install a new DDC panel inside the Electrical room 2nd floor of Bldg.
#54 and it shall be based on BAC net/IP and shall be compatible with existing IT equipment inside
communication room of Bldg. # 54.
 CONTRACTOR shall connect the new DDC panel to the existing communications closet in the
combination room of Bldg. # 54 using CAT -6 Ethernet cable.
 CONTRACTOR shall supply and install new power cables and conduit for the new FAHU and
Exhaust Fan (one duty + one normal) on the Roof as shown on the drawing HVAC Roof Power,
see project drawing JHAH-DD-DP-010. Contractor shall verify the exact cable routing at site
from the Roof to the existing Panel board PP-3A located at Electrical Room 395 of the third floor
Dental Building 54.
 CONTRACTOR shall supply and install wires and conduits of sizes and types as indicated on plan,
including the necessary pull boxes, boxes and fittings as required and all accessories to complete
the installation of the electrical wiring system as shown on drawing JHAH-DD- DP-010.
 CONTRACTOR shall supply and install disconnect switches with the ratings as indicated on plan,
including the necessary wiring, conduits and all related accessories to complete the
installation of the electrical power system as shown on drawings JHAH-DD-DP-010.
 CONTRACTOR shall supply and install new circuit breaker (30AT/100AF) in spare
compartments no. 26,28 and 30 of existing Panel board PP-3A located at Electrical Room 395 of
the third floor Dental Building 54 including all appurtenances and accessories required to
complete the installation of power distribution system as shown on project drawing HA-
114201.004 Rev. 01C.
 CONTRACTOR shall supply and install new circuit breaker (100AT/100AF) in space
compartments no. 5L of existing Control gear CG-1 including all appurtenances and accessories
required to complete the installation of power distribution system as shown on project drawing
HA-114197.002 Rev. 01C

HVAC Demolition Works:

 CONTRACTOR shall dismantle/demolish all existing supply, return and exhaust air outlets,
dampers, flexible ducts and accessories that are affected by the new work at IRC area as
shown on project demolition drawing and without affecting the other spaces. Main
supply/return/exhaust ducts at IRC area shall remain as it is.
 CONTRACTOR shall remove and salvage all existing electric duct heaters, and associated
accessories serving that are affected by the new work at dental lab area as shown on the
project drawing and without affecting the other spaces
 CONTRACTOR shall demolish existing refrigerant piping, valves, and accessories from condenser
unit, to be salvaged back to the nearest main.
 CONTRACTOR shall close all walls and roof openings of the existing HVAC system, which shall not
be in use for the new HVAC system. Also seal, weatherproof, all the other unused openings.

New Works:

 CONTRACTOR shall supply and install Central Fresh Air Handling System (FAHU), hygienic
type, modular double skin exposed type shall be provided for the dental lab area. Units shall
consist of intake box, pre-filter, final filter, chilled water cooling coil with belt driven fan/motor,
HEBA filter, condensate drain pan and all necessary accessories. FAHU units shall be located on
roof as shown on the drawings, and in accordance with project specifications, Saudi Aramco
standards and related international standards.
 CONTRACTOR shall supply and install Exhaust Fans with capacities as indicated in the
equipment schedules along with disconnect switches, auto changeover switches, vibration
isolators and all necessary accessories for satisfactory operation as shown on project
drawings. Exhaust Fans serving hazardous exhaust areas shall be installed with 45° mounting
angle and ductwork for discharge not less than five (5) meters above rooftop.
 CONTRACTOR shall supply and install Electric Duct Heaters (EDH) air terminal units, UL Listed
with capacities indicated in equipment schedules complete with air flow switch, controllers,
room sensors and associated accessories as shown on the drawings. CONTRACTOR shall seal
all ductworks with approved duct sealant for air tightness.
 CONTRACTOR shall furnish and install supply, fresh, exhaust and return metal ductwork complete
with fittings, sealant, hangers and supports as shown on the drawings, and SMACNA latest
edition.
 CONTRACTOR shall supply and install aboveground chilled water pipes, ERW black steel pipes,
schedule 40, conforming to ASTM A53 grade B, pipes & fittings, Insulation and aluminium
jacketing for exposed pipes including supports and necessary valves & fittings, insulation and all
accessories.
 CONTRACTOR shall supply Aboveground Chilled water pipes ERW black steel pipe, schedule
40, conforming to ASTM A53 grade B, pipes & fittings, Insulation and aluminium jacketing for
exposed pipes including supports & necessary valves & fittings, insulation and all accessories as
per drawings and in accordance with project specifications, Saudi Aramco standards, JHAH
requirements and related international standards.

FINAL INSPECTION, TESTING AND COMMISSIONING

 CONTRACTOR shall provide pre-start-up and commissioning of HVAC units, as per SAES- K-001 in
accordance with project specifications. CONTRACTOR shall perform leak testing for all ductwork
prior to insulation in accordance with SMACNA and ASHRAE.

CONTRACTOR shall test, adjust and balance the HVAC systems as per SAES-K-001, ASHRAE
and SMACNA and to the capacities as shown on project drawings and construction
specifications. TAB shall be performed or supervised by a NEBB certified engineer/personnel
and in accordance with project specifications, Saudi Aramco standards related international
standards.

SECTION 4: HAZARD IDENTIFICATION PLAN

4.1 HAZARD IDENTIFICATION PLAN


 To minimize or prevent injuries to personnel and damage to properties.
 To warm the general public for any potential Hazards that constitute from the work to be done
 Proper planning and execution of works in safely manner
 To establish what safety tools, materials and personnel protective equipment

4.2 HAZARD CLASIFICATION

Hazards are classified into following categories

Class “A” Hazard: A condition or practice likely to cause permanent disability, loss of life/ body part
and / or extensive loss of structure, equipment materials.
Class “B” Hazard: A condition or practice likely to cause serious injuries or illness (resulting in
temporary disability) or property damage that is disruptive, but less severe than
Class “A”
Class “C” Hazard: A condition or practice likely to cause minor (non-disability injury or Illness or
. Non-Disruptive property damage)

4.3 HAZARDS SUMMARY


The project has been evaluated in terms of the above three (3) hazard classes and specific hazards in each
class have been identified. A corrective action is recommended for each hazard. Personnel working on this
project will be notified that hazards, created by others, could be present.

 TEMPORARY & PERMANENT ELECTRICAL INSTALLATION CLASS “C” HAZARDS


Distribution of electricity on a construction site is different from a permanent installation. Hazards will arise
when unskilled or unqualified electricians carry out electrical work. The use of lockout and tags may be
required on certain operations. The use of GFCI’s mandatory on this project.
 WELDING, CUTTING & BRAZING CLASS “A” HAZARDS
Welding, cutting and brazing are safe operations if carried out in a correct manner. Where equipment is
defective, or there is no arranged plan, hazards can arise. BLACK ARROW will ensure that all equipment is
correctly maintained and thoroughly checked before use.

 HAND/ POWER TOOLS CLASS “B” HAZARDS

Power tools allow many jobs to be carried out more efficiently, and with greater speed and accuracy.
Hazards will arise when unskilled and untrained workers are allowed to operate hand /power tools. BLACK
ARROW will ensure the only trained personnel operate these tools. Step will take to ensure equipment is in
good state of repair.
 SANITATION & HOUSEKEEPING CLASS “C” HAZARDS
A major factor influencing the health of individuals, where public sewers are not available, is the proper
disposal of human waste. Many diseases of various types can be transmitted from one person to another. For
this reason, BLACK ARROW will make every effort to prevent, or control this hazard.
 SCAFFOLDING / LADDERS CLASS “A” HAZARDS
Competent, trained workmen will erect scaffolding in accordance with the Saudi Aramco Black Arrow
construction safety manual. Careful erection and inspection of scaffolding is essential to prevent accidents.
All ladders used on this project will meet ANSI standards.
 EXISTING ABOVE / UNDERGROUND UTILITIES CLASS “A” HAZARDS
Areas where work has to be executed can have different types of above / underground utilities, such as
power cables, pipelines corridor, telephone cables, and gas line. The hazards arise by damaging any of these
services.
 CONCRETE FORMS & SHORING CLASS “A” HAZARDS
This section covers several areas of work, all of which present hazards. The placement of concrete, vertical
shoring, from work & shuttering, are but a few of the concerns. Accident can be caused by employee’s
action or inaction’s, and / or unanticipated failure of mechanical devises, concrete forms, or shuttering.
Procedures outlined in the Saudi Aramco Black Arrow Construction safety manual will
be followed.
 MECHANICAL EQUIPMENT CLASS “B” HAZARDS
Mechanical equipment improves the quality, and efficiency of construction work, but equipment operators
can cause hazardous situations. BLACK ARROW will ensure specifically trained and qualified operators for
the types of equipment they are operating. All equipment must be well maintained and regularly inspected.
 CHEMICALS CLASS “A” HAZARDS
Any chemical should be treated as hazardous. Handling, storage, identification and labels which are
incorrectly tagged, and spillages can cause extremely hazardous conditions. BLACK ARROW will follow
procedures as outlined in the JHAH/Saudi Aramco Construction Safety.

12) Personal protective equipment (PPE)
 When a hazardous situation is recognized, steps shall be taken to eliminate the hazard by use of
engineering or administrative controls. If engineering and administrative controls do not adequately
eliminate the hazard, PPE.
 Shall be used to protect personnel from the hazard.
 At a minimum, hard hats, safety glasses and safety shoes shall be worn in SA restricted areas and
project work sites. Additional PPE shall be selected and used based upon the results of a documented
PPE needs analysis, which shall consider the exposure hazard(s), materials handled, and activities
performed by personnel. Contractor’s documented PPE needs analysis shall be made available to the
SA proponent organization (SAPO).
 Supervisors shall ensure their personnel are trained in inspection, use, maintenance and storage of
PPE according to the manufacturer’s instructions and this chapter.
 It is not sufficient just to provide PPE at the workplace. The BLACK ARROW shall ensure it is
readily available and accessible and clear instruction is given on where to obtain it. In circumstances
where no other measures can give adequate control of the risk, then the PPE that would give the best
protection that practicable in the situation must be provided. PPE shall meet applicable ANSI
requirements or equivalent as specified in writing by the SA Loss Prevention Department.
 Proper PPE (e.g., correct for type of exposure, sized to fit the worker, meets applicable standards, no
defects) shall be provided to personnel when/where required.
 PPE shall be visually inspected by the user before each use. Defective or damaged PPE shall not be

 At toolbox meetings, shall be performed at the work site. In addition, proper emergency response using
the Saudi Aramco Standard procedure for Calling “911” text below shall be posted in conspicuous in the
area to guide employees during emergencies.
 Provide minimum of (2) assemble points with a minimum separation of (30) meters away from the work
site.
13.1 Credible Scenarios:
FIRE
 When fire alarm is sounded all non- essential personnel shall proceed immediately to the nearest
emergency assembly areas while all emergency response personnel shall proceed to the incident
command post.
 The incident commander and the fire brigade team leader will assess the situation. If the fire
incident is controllable the incident commander will give the order to the fire brigade team leader
together with his team member to contain the fire.
 If the incident is un- controllable the incident commander will give order to the emergency
coordinator to contact emergency response organization by dialing 911/998 or following the
procedure on reporting emergency for proper assistance. Finally, if the fire is contained a fire watch
will oversee the site for possible re-ignition that will result to another fire.

EMERGENCY REPORTING

When reporting an emergency speak slowly, calmly, distinctly, in a clear voice and give information in the
following sequence:

Dial 911 from the nearest telephone.


Call the relevant contact number if busy call next alternative.
Then clearly state:
I have emergency to report:

 My name ___________________________
 My badge no. is_____________________________
 I am calling from_______________________

 I see a _____________________ (fire, gas leak, traffic accident) (location)


 There are _________ number of people injured and required medical attention
 Stay on the line answer any further questions and once finished return to the accident scene if you are
designated with and emergency response task such as firefighting, medical assistance or rescue activity.

 If you are not assigned an Emergency Response task, then stay in the assigned ASSEMBLY
AREA.

The emergency response is started by the first person that observes the emergency to contact his
immediate supervisor.

It is the responsibility of the affected supervisor to broadcast the initial emergency telephone of

13.2 SITE PROJECT EVACUATION

In the event of an accident along any of the access roads, including the main Roads from to work sites, or at
a work site or remote area the person arriving at the scene shall do the following:

 Render necessary and immediate first aid.


 Call the Incident Commander if he is not available dial 013-673-9110
 Identify the location of the accident.
 Give full name, badge number and a contact phone.
 State if there are any injured persons and how many
 Repeat all relevant critical information such as; accident location, injuries and how many, etc.
 Ask Security Control Dispatcher to repeat the information to make sure it is complete and correct.
 In the low probability of a site project evacuation, the instruction shall be given from Black Arrow
crisis cell. Once it is confirmed.

 Buses
 Project cars
 Ambulances for the injured personnel.

 Evacuation will have to comply with the Site Security Plan. In the event an emergency or risk to
Black Arrow personnel, such as war or terrorism and an evacuation order is issued by Black Arrow.

13.3 COMMUNICATION

In case of an Emergency, Mishap, or Disaster; all workers are to be familiar with the respective jurisdictions
and notify the designated Incident Commanders as follows:
