RFP1131 Att 02 Application Template
This template document must be completed and submitted with your application in
accordance with the submission requirements outlined in the RFP. Use this required
template to prepare responses to the questions found in the Request for Proposals
(RFP). It is highly recommended you save this document frequently while drafting your
responses to avoid the potential loss of work.
Please note: It is the responsibility of the applicant to ensure they are entering
complete and accurate responses into the correct fields. Please double-check all
information submitted in your proposal. In the event of any discrepancy or
disparity between the RFP document and Attachment 2 – Application Template:
the RFP document will always take precedence.
Please note: Eligibility Questions and Administrative Questions are not included in
this Application Template but remain a required component of your application and will
be completed via an online form. Please see the RFP document for additional
information.
The proposal will include responses to the following questions, which are taken from
RFP Section 5.4 Proposal Content. Please be succinct yet thorough in your
responses, being sure to answer each component of the questions in a manner that
clearly describes your proposal to reviewers. Responses to each individual question are
limited to no more than 5,000 characters.

A1. Describe the planning activities conducted that shaped how the 2
proposal was developed. Proposals must include the details of a
community needs assessment that was conducted and a summary of
the findings. Include the resources used and any community or social
trends that support the demand for the proposed after-school program
services. Examples of trends include, and are not limited to, supporting
the mental health of students, the increased need for after-school
programs in rural communities, and expanding pathways to careers
outside of traditional higher education.

program will support students’ physical and emotional health as they
recover from the social isolation impacts of COVID-19 and reengage in

I3. Identify how expenses for the proposed project are reasonable, 5
necessary, allowable, and are realistic based on the award amount
requested.
I4. Describe how the budget focuses on program expenses and direct 5
services rather than administrative costs. Clearly identify the amount
of funds being proposed for indirect costs and provide a description
of the expenses. Include a copy of your approved Federal Indirect
Cost Rate Agreement with your application, if applicable.
I5. Please indicate the annual dollar amount in OCFS funds being Number

I6. Please clearly identify how the total annual funding request was Text
