    1. Favorable recommendation of the Presiding Judge


    2. Indorsement of the Executive Judge
    3. Certification under oath by the Presiding Judge that the list submitted contains the
    names/ s of all the applicant/ s who applied for the position pursuant to OCA Circular No.
    74-2010
    4. Certification under oath by the Clerk of Court that the position to be filled has been
    posted in three (3) conspicuous places
    5. SPB LC Forms required under OCA Circular No. 128-2016
    a) Recommendation Form
    b) Interview Form
    c) Examination Results Form
    6. Four (4) copies of properly, completely and recently accomplished Personal Data Sheet
    including additional Work Experience Sheet;
    7. Two (2) Authenticated copies of the Civil Service Eligibility
    8. Authenticated copy of the Transcript of School Records
    9. Sworn Affidavit of no incumbent relative(s) within the third degree of consanguinity or
    affinity in subject branch/office
    10. Copy of proof of completion of relevant Training / Seminar
    11. Original Philippine Statistics Authority (PSA) copy of Birth Certificate
    12. Original copy of latest NBI Clearance
    13. Certification of employment from previous offices
    14. Latest Medical Certificate
    a) Hematology
    b) Microscopy
    c) Fecalysis
    d) X-Ray
    e) Drug Test Report
    f) Neuro-Psychological Report
    15. Affidavit to accept appointment under temporary appointment if the Office of the Court
    Administrator found that I do not meet the requirements for the position.

    All the documents I submitted are complete and up to date. I am solely responsible for any
    incomplete or out-of-date documents or data. I used the latest version of the Personal Data
    Sheet and all the fields therein were accomplished.
