9:30 AM – 11:30 AM – Consultation
Noon – Lunch Break
1:00 PM – 3:00 PM – Continuation of consultation
3:30 PM – Departure from DOST Region V
5:00 PM – Arrival at Ligao City
We confirm that we have reviewed the itinerary above. We understand that the designated
companions will pick up our children from Ligao National High School, Guilid, Ligao City, and
accompany them back within the approved schedule. As parents or legal guardians of the students
mentioned above, we acknowledge that we are aware of the research activity our children will
conduct in fulfillment of the requirements for their Research IV subject. We give our full consent for
our children to conduct said research activity.

We have considered the benefits that our children will derive from their participation in the
activity, with the understanding that every precaution will be taken to ensure our children’s safety.
We have also ensured that our children understand the importance of their safety and that they
must obey all rules and regulations given by the school authorities and supervisors for the safety of
the group.

To the extent allowed by law and rules, I hereby agree to waive, release, and discharge all
other claims, causes of action, damages, and rights against the Ligao National High School,
___________________, and its personnel relative to the conduct of the research consultation.
