AFFIDAVIT OF INSUFFICIENT INCOME

THAT I, JANE EILEEN A. CAOILE, of legal age, Filipino citizen and with residence and
postal address at 73a RIVERA COMPOUND BARANGAY MOONWALK PARAÑAQUE CITY,
after having been sworn to in accordance with law, hereby depose and say:

That, I am the mother of DAVID SIMONE A. CAOILE, male, 18 years old, born on DEC 07
2005 ;

1. I work as a Family Housekeeper while my husband on the other hand has left us
and I have no communication with her ever since.

2. my monthly income as a housekeeper is Php 5000 and is not sufficient to support

the need for education of my son and finance other required expenses;

3. That I am executing this Affidavit of Insufficient Income for the application of my

child to SPES; and

4. That I am executing this Affidavit of Insufficient Income to attest as to the veracity

of all the above facts and for all legal intents and purposes this may serve.
