Subject: Change of Name and Gender.
I am writing on behalf of my client, Mr. Ayush Singh.G (previously
Ms.Shruthi.G), aged about 32 years, R/at No. 04, 3rd Floor, 4th Cross, 5th Main,
Sampangiramanagara, Bangalore South, Bengaluru, Karnataka – 560 027. I hereby
would like to bring to your attention that my client recently underwent “Gender
reassignment surgery”. In essence, my client was born as a female but since they did
not feel comfortable being in the body that was assigned during their birth, they
decided to undergo a surgery changing their gender from female to male.

To give a brief about my client, they were born on 01/06/1991 at Kukanoor, Koppal
district and they are the last child of the four children that was born to my parents. My
client state that they have completed my 1st standard to 7th standard at Sri.
Buvaneshwari aided higher primary school and 8th to 10th studied in Sharada Nikethan
English Medium high school and both are situated at S.N.Pete, Kampli, Bellary
district. Furthermore, my client completed their I & II PUC from Vijayanagara
College, Hospet and completed Bachelor of Computer Application from Gulbarga
university and M.Sc in computer science from Vijayanagara Sri Krishnadevarya
university in Bellary.

My client state that, for most part of their life they were raised in the Hyderabad-
Karnataka region and as such the Assistant Commissioner, Hospet sub division had
issued a regional certificate in their favour as envisaged under Article 371-J. In
addition, the Tahsildar Kampli Taluk had also issued a caste cum income certificate
along with domicile/ residence certificate in their favour.

It is pertinent to state that, my client underwent surgery for removal of breasts at

Designer body, Mumbai on 31/08/2021 and subsequently, They underwent “Total
Laproscopic Hysterectomy with Bilateral Salphin Goophorectomy” at Kokilaben
Ambani Hospital, Mumbai. Finally, my client underwent Phalloplasty in order to
replace their genitals.

It is trite to state, ever since my client underwent the aforementioned surgeries they
have been living as a man and they now also identify as a man. Nonetheless, all my
clients documents vis-à-vis school and college marks card, AADHAR, PAN card, etc.,
reflect their name as Ms. Shruthi.G as they were a woman before. Now that my client
gender has been reassigned/changed, it is necessary that the gender and the name in
my client SSLC marks Card is also changed to Male and Mr. Ayush Singh.G.

My client state that they are currently unemployed though they are very well qualified
to get an employment. It is pertinent to mention that, most work places ask for
personal details such as AADHAR card and marks card to give an employment but
given my clients story, nobody believes that they have undergone gender reassignment
surgery. My client is even depraved of the basic necessities as they are unable to get a
job. That apart, my client are finding it difficult to travel and stay in places as most
places in Bangalore ask for some kind of Identity card and they are unable to provide
the same as all the documents that are in my clients possession are in respect of
Ms.Shruthi.G. In light of the above, I humbly request your good self to consider my
clients representation and effect change in their gender and name in SSLC marks
Card.

I’m herewith enclosing the SSLC marks Card and AADHAR Card of my client for
your perusal.

Therefore through this legal notice I kindly request you to make the necessary change
in the respective documents of my client. Thank you for your attention to this matter.
We trust that a swift resolution to my client’s problem can be reached. Your
Cooperation is appreciated.
