AFFIDAVIT

Herein I Deepa W/o Raju Doddabommnnavar, Age: 23 years,

Occ: Housewife, R/O. : Bhavaninagar Belagavi, today at Belagavi
do hereby state on solemn affirmation and make this affidavit as
under:-

1] I am the plaintiff in this suit and I know facts and circumstances

of this plaint hence I am competent to swear this affidavit for
myself.

2] That, I am the permanent resident of above mentioned address

and I have filed this suit against the defendants for the relief of
declaration and I have narrated entire facts in the plaint the same
may be treated as peace and parcel of this affidavit to avoid
repetition.

3] Further I respectfully submit that, my name in the death

certificate of my husband is wrongly mentioned as “Rani alias Uma”
instead of “Deepa” in column No.7 of the certificate. The said death
certificate is issued by the defendant. Hence, the matter may be
treated as urgent and as such for getting Warsa certificate.
Therefore, I constrained to file this application to dispense to issue
notice to the defendant, if this application is allowed no loss or
prejudice will cause to the defendant on the contrary I will be put to
untold hardship and loss.
Therefore it is most humbly prayed that, issuance notice to
the defendant prior filing of this suit may kindly be dispensed to
meet the ends of justice.

The contents of the above paras are read over and explained to me
in my vernacular and the same are true and correct to the best of
my knowledge and belief.
