GUIDELINES FOR RENEWAL and NEW APPLICANTS OF CPA ANNUAL

VEHICLE PASS (AVP)/ STICKERS

Renewal and New Applicants

All companies with approved Permit to Operate (PTO) /Annual Access Fee (AAF) and AVP application/s for
the current year may process for new/renewal of application.
Application for renewal of AVP Stickers shall commence as early as October 01 of the current year. All
interested companies without an approved PTO/AAF and AVP may apply as new applicants.

Requirements for AVP / Stickers

1. Four (4) copies of accomplished application for AVP (FM-PSD-01) duly signed by the owner/manager (with
attached company and valid ID). Entries on the form shall be typewritten and arranged according to the
listing of the LTFRB Order of Exemption or LTFRB Franchise.
2. Photocopy of Permit to Operate (PTO)
3. Photocopy of LTO CR and updated OR of vehicle registration;
4. For Private Service Vehicles registered under the name of the company’s employee:
a. Certification from the employer that the registered owner is a bonafide officer/employee of the
company authorized to transact for the company’s business inside the port area.
b. Photocopy of the employee’s company Identification Card (ID).
5. For Prime Movers/ Tractors entering ISPS Zone, applicant must attach or submit a letter request addressed
to the office of the PSSEMD upon approval of AVP application;
6. For vehicle/s not registered under the company or proprietor’s name, please attach supporting documents,
e.g. a) Notarized Deed of Sale of Motor Vehicle (MV) must not exceed 1 year from the date of purchase
from the first owner upon applying for CPA sticker; b) Notarized Contract of MV is rented/leased; c)
Marriage Contract if under the name of the spouse; d) Birth Certificate if registered under parents or
siblings; e) Notarized Affidavit of consent to use as service vehicle will be required, etc.

AVP / Stickers
2 – 4 wheels - Php 224.00* per vehicle (*All costs 12%
6 – 10 wheels / Prime Mover - Php 336.00* per vehicle VAT inclusive)

Note: Application must be placed in a folder with fastener

Period for renewal is from ______________. All AVP stickers are valid until the end of the current year. All
applications submitted after the specified deadline is subjected to 20% surcharge on AVP charges. For inquiries,
