ALS: US ENTITIES ATTENDANCE POLICY
An absence is defined as a failure to report and remain at work as scheduled; this includes late
arrival at work and leaving early as well. Absence includes all time lost from the job whether
excused or unexcused, avoidable or unavoidable.
You are expected to report to work on time, on a regular basis. Unnecessary absenteeism and
lateness is expensive, disruptive, and places an unfair burden on other employees and your
manager or supervisor who must compensate for an employee’s absence. Unsatisfactory
attendance will also result in disciplinary action, including possible suspension and termination.
If you are going to be late or absent for any reason, you are required to personally notify your
manager or supervisor as far in advance as possible but no less than 1 hour before your
scheduled start time on the day of the absence via telephone only. An explanation of why you
are going to be late or absent and when you expect to return to work is required. Your manager
or supervisor must also be notified prior to your leaving work at other than regularly designated
times. Employees who fail to call in and have not reported to work for three (3) consecutive days
will be deemed to have abandoned their employment and thus risk termination, unless they are
on an approved leave of absence.
It is your responsibility to ensure that proper notification is given. Asking another employee,
friend, or relative to give such notification is not acceptable, except under emergency conditions.
Communication of the absence must be made via telephone to your manager or supervisor or
his/her Manager. Text messages, voicemail or e-mails are unacceptable. An employee is
expected to follow appropriate call-in procedures for each day absent unless the absence has
been previously approved. Any employee who fails to give such notification will be charged with
an unexcused absence.
If you are absent because of an illness, injury, or surgery for three (3) or more consecutive days,
you are required to submit a written release from your attending physician stating you are able
to resume normal work duties before you will be allowed to return to work at the manager’s or
supervisor’s discretion.
The Attendance Policy applies to all ALS employees effective November 1, 2018 and is applicable
after an employee has exhausted all allotted Paid Time Off (referred to as PTO herein after) hours
available or fails to comply with the ALS USA Employee Handbook 2018, section 5.3 Business
Hours & Attendance. The policy is intended to ensure that employee attendance and
absenteeism is managed in a transparent and consistent manner.
ALS MOTOR VEHICLE POLICY ACCEPTANCE
All staff assigned a company vehicle or who choose to enter into a novated lease contract
administered by the Company are required to e-Sign and complete this task.

As a condition of my employment with ALS I, agree to provide samples of my blood for the
purposes of testing the level of lead present. I understand that the results of the blood test will
be used to monitor my safety whilst working at ALS and consent to the release of the results to
the Company Doctor or Medical Practitioner. I understand that if the results of the blood tests
indicate lead higher than levels considered safe under the National Standard for the Control of
Inorganic Lead at Work [NOHSC:1012(1994)], I will be required to undertake further blood
testing and participate in an investigation of my work practices and/or my work environment.
Should it be found that my work practices are not compliant with safe work instructions for Fire
Assay, I understand that I may be subject to a formal disciplinary process as outlined within the
ALS Certified Agreement. Non-compliance with safe work instructions also includes the failure to
attend blood tests, whether deliberately or unintentionally, and continued failure to attend tests
may result in disciplinary action.
I agree to follow all control measures implemented to minimise my exposure to lead. This
includes removal of facial hair that may inhibit the performance of the half face respirator, the
use of other personal protective equipment, or periodic removal from the fire assay area as
directed by Management. In the event it is required that I be removed from the fire assay area, I
acknowledge that it may not always to possible for Management to provide me with alternate
work and as such, I may be required to take periods of paid or unpaid leave from work until such
time as I am able to work back with the fire assay area again.
If, during the course of my employment, I receive a blood test result that exceeds the level
deemed safe by the National Standard for the Control of Inorganic Lead at Work
[NOHSC:1012(1994)], I acknowledge that Management will be informed of this.
I understand that ALS will maintain copies of blood lead level results on my personnel file, and
that these results will additionally be used for statistical reporting purposes.
