your bank bearing Account No. 677667422201. Bank authorities issued Pass
Book and maintained as well as norms.

Here I am requesting you sir, while I shifted my house I lost my pass

book, kindly issue new pass by cutting service charges in my account balance.

And I request you to issue last six months statement of my account.
