Formative Assessment – Child-friendly rating scale

Learning Objective: After a lesson on defining what rules are and what it means to follow and break rules
by creating an anchor chart, students will be able to describe and give an example of each.

Assessment Description: In this assessment, students will be assessed on their ability to describe what a
rule is, give an example of following the rules, and an example of breaking the rules. Students who use
the first scoring guide will write their answers, while students who use the second scoring guide will
verbally answer.

Name_____________________________________________

A rule is _________________________________________________________________
_________________________________________________________________________.

An example of following the rules can be when

___________________________________________________________________________________
__________________________________________________________________________________.

An example of breaking the rules can be when

___________________________________________________________________________________
__________________________________________________________________________________.

Rationale: The assessment aligns with the learning objective because it has students define and give
examples of what a rule is and the two different kinds.

Scoring Guide Description: This scoring guide serves as a child-friendly rating scale for students to
complete after they have finished the assessment. The purpose of the rating scale is to help students
understand where they are with their learning and how they are feeling after answering the questions.
The rating scale also helps the TC see where the students are and how they are feeling about the
content.
