PROJECT HIGHLITS:

Title of the content: SHE EMERGENCY RESPONSE PLAN FOR BRIDGE No: 92.

1. CONTRACT NUMBER: HQ/EN/WC/Bridges/D&B/1/SOMA

2. LOCATION OF THE PROJECT SITE: Bridge no-92 and DFCC western Freight Corridor Project Design and
Construction of Important and Major Bridges (54Nos) of Western Freight Corridor between Vaitarna and Utran on Vasai-Bharuch
Section.

3. PERIOD OF THE PROJECT: Four years

4. EMERGENCY RESPONSE PLAN PROFILE:

Experience of years have shown that accident, dangerous occurrence, emergency or


disaster occurs at any time and we lose life, property, production/progress of the
project…. etc, If we do not fully prepared to control over and minimize it. Therefore
it is most advisable to foresee such situation and anticipate the unsafe conditions of
the work place & unsafe actions of the workmen which are controllable main root
causes (98%) of most of the accidents and emergency. Natural Calamities is
responsible for 2% emergency but it offers disaster.

To control unsafe condition, first think about engineering means to rectify it


whenever it is possible. It offers permanent solution and makes the system full
proof. Give last priority to PPEs to rectify the unsafe condition & unsafe action
because it is temporary solution to cure the total loss prevention.

To control unsafe actions of the workmen makes induction procedure effective,


conduct training & refresher workshops group wise of the workmen on the subject
for they assigned for. Motivation & promotional schemes are powerful tools to
control unsafe actions of the workmen. Practice of Mock drill for different type of
emergency helps to find out the deficiency of the emergency response plant &
makes it more effective. Ultimately it reflects on enhancing awareness of the
workmen & other employees too.

Participation of the senior management personnel in the safety movement & follow
the safety rules at site constitutes the example for the workmen & enhancing
standards of the SHE requirements.

Natural calamities (2%) like earthquakes, cyclones, floods, forest fire are natural
phenomena & it causes disaster. It is not controllable by the human beings, it is
catastrophic situation in which day to day life of the people suddenly disrupted and
people are plunged in to helpless situation. We can able to detain some damage or

minimize effectiveness if we prepare emergency/ contingency plan for the same &
educate the employees for combating such situation.

Safety is ever body responsibility & together we can achieve Zero Accident target &
develops safety culture in the organization.

5. PURPOSE:

The purpose of the Emergency Plan is:

5.1 To protect persons and property of the company in case of accidents,


dangerous Occurrences, emergencies and disasters caused.
5.2 To inform surrounding people about emergency occurrence if it is likely to
affect them.
5.3 To inform authorities including helping agencies i.e. Hospitals, Fire Fighting
Station, Police, Transporter …etc. in advance.
5.4 To identify, assess, foresee and work out various kinds of possible hazards,
places, having Potential and damaging capacity in case of abnormality
happens.
5.5 To work out a plan with detailed instructions to cope with abnormality
happenings base on Available resources.
5.6 To add necessary requirements for rescue operation if it is not already
available.
5.7 To establish machinery for rescue and recuperation operations, total loss
control and Prevention of harms and avoid recurrence of abnormality.
5.8 To ensure that absolute safety & security are achieved within the shortest
time.

6. OBJECTIVE:

6.1 The purpose of this document is to identify and control “On Site Emergency”
measures.
6.2 To establish “On Site Emergency” management system and responsibilities.

7. SCOPE:

This plan is applied for construction of the Bridge No: 92

8. RESPONSIBILITY:

8.1 Site Project Manager of Reach-1.


8.2 Sr. SHE Manager of Reach-1.

9. GENERAL CAUTION:

General Caution to be taken at the time of emergency as mentioned below:

9.1 As soon as emergency is noticed by the inventor, he tries to control it in case


of the Fire using extinguisher or “Switch On” the Siren and alert all workmen
& staff members of the site. The Siren can be activated by anybody working
at site.
9.2 The employees have not assigned particular job for combating emergency,
will Assemble at assembly point & act as a supporter whenever they are
asked, but do not create any hurdle or barrier.
9.3 The area to be cordoned off & main access shall be closed to avoid entrance &
Interruption of the outside public. The security guards will control the entry &
exit.
9.4 The visitors will be requested to leave the site immediately by the DSIC.
9.5 The communication system shall not be engaged unnecessarily.
9.6. All hot works will be stopped & gas cylinders will be closed & shifted to safe
place.
9.7 Switch off electrical supply, starts generator if supply is required.
9.8 Remove the fire proven materials from the site & store it safe place.
9.9 Inform the external agency i.e. Disaster Prevention & Management Center
(DPMC) for combating emergency. The Railway authorities, Police, Hospital &
District Magistrate if emergency affects to the general public living in the
vicinity of the project site.
9.10 Keep access clear for the DPMC tender & rescue team, lead them up to
emergency point & act as a helping hand to expedite their efforts.
9.11 Do not get panic & spread rumors. Be serious & avoid teasing or horse playing
at the time of emergency.

10. MAP OF CONSTRUCTION AREA:

The MAP of construction area is attached herewith for projecting location of the
Bridge: 92 site & other important locations i.e. Gantry, Office containers, Assembly
Point, Siren, Extinguishers Post…etc... It also projects location of rescue equipments
Like Floating Platform (BARGE), Tug Boat for reference at the time of emergency
Arises.

11. EMERGENCY MANAGEMENT ORGANISATION & FUNCTION IN BRIEF:

Minor accidents do not cause much harm but major accident due to emergency or
disaster cause great harm of person, property and production/progress of the
project. The emergency situation arises all of a sudden and contains high potential
to cause havoc. Therefore such situations/chances and risks should be thought in
advance and It should be planned beforehand and control it within the shortest time.
Emergency preparedness and response for the Bridge No-92 site or “On Site
Emergency Response Plan” is prepared after considering the occurrences of various
emergencies as follows:

 Fire and Explosion.


 Fall of person in the River Water.
 Lifting appliances and equipment collapse or drop in the River water.

 Emergency due to spillage of oil or construction material in to the river


stream.
 Emergency due to sudden release of water from different rivers canals &
barrage…etc.
 Collapse of A- Frame or guide frame or damaged of gantry.
 Natural calamities like Heavy rains fall, Cyclone, Flood, Forest Fire,
earthquake ...etc.
 Criminal or Terrorist attack.

All emergency (incident / accident) situations which occur on the Bridge No-92 area
will be attended as per Emergency Response Plan & reported to the DFCC and SPAN
Management. Reporting will be done telephonically first & by written within 24 hours
as outlined in the DFCC SHE manual clause No 14.1, 14.1.1 & 14.1.2.

11.1 On Site Emergency Combat Teams:

Depending upon the type of the emergency, site emergency combat teams will take
action & control the situation.
In order to combat emergency in time & manage the situation in a systematic way
at site,” On Site Emergency combat teams” are formed for the Bridge: 92 & team
member responsibilities are defined. These teams are:

 Fire Fighting / Rescue team: members are Mr. Ram Milan (Leader),
Mr.Hadikul Islam Mr.Parash Sahni, Mr.Ghansyam, Mr.Madhuban Shahni
Mr.Ameerul Huck, Mr.Ramu Shahni Mr.Nand Lal, Mr. Ram Dheeraj Mr.Rubil
Shaik Mr. Rohi Dash
 Engineering Team: members are Mr. Raja Babu (Leader), Mr. Nilesh, Mr.
Natraj , Mr.Alimili Mahesh.& Anjanikant Guptha
 First AID Team: members are Dr.Anil Kamath (Leader) Dr.Birendra Bahadur
Singh.
 Administration Team: members are Mr. M. Manoj Kumar (Leader),Mr. Ajay
Surkar, Mr.R.Jagadish, Mr.Fulla Reddy.
 Security Team: Members are Mr.Sandeep Gupta (Leader), Mr.Pramod Kumar
Mr.Sakil Salmani, Mr.Pudleep Bharat Mr. Ramesh Goal.

Above mentioned team will be led by the Main Incident Controller (MIC), Dy.Incident
Controller (DIC), Site Incident Controller (SIC) & Dy. Site Incident Controller (DSIC).

The Deputy General Manager of Reach-1 Mr.VC.Shivanand & Mr. N S .Kulkarni


(Sr.Manager-Civil) is assigned for Main Incident Controller (MIC) & Dy.Incident
Controller responsibility. The engineer-Civil Mr.Alimili Mahesh & Mr.Nilesh
(Eng...Mech) are assigned for Site Incident Controller (SIC) & Dy. Site Incident
Controller (DSIC) respectively.

11.2 Responsibility of Incident Controllers:

 Responsibility of Site Incident Controller: Whenever the Siren is wailing


by the emergency (Accident/Incident) inventor. The Site Incident Controller

(SIC) has to take control over the situation & start the operation by activating
rescue team & mobilize other resources to suppress the emergency.
 Responsibility of Dy. Site Incident Controller: To assist the Site Incident
Controller (SIC) & pass on the information to Sr. Manager-SHE about
emergency & keep in contact with SIC & DIC for requirements from the store.
 Responsibility of Dy. Incident Controller: As soon as information is
received from the Dy. Site Incident Controller, the Dy. Incident Controller
shall rush to the spot & assess emergency situation. Immediately reports to
Main Incident Controller. He gets advice from him & makes strategy to
combat emergency. He acts as a coordinator.
 Responsibility of Main Incident Controller: The Main Incident Controller
(MIC) will make available in the Emergency Control Room as soon as message
received from the Dy. Incident Controller. He assesses the gravity of incident
& guide the DMIC for combating emergency. If situation becomes out of
control the Main Incident Controller (MIC) is authorized to declare emergency
& involved external agency for combat- ing the emergency. He gets declared
emergency by instructing the DMIC & SIC.
The Main Incident Controller (MIC) will report to DFCC telephonically first and
by written within 24 hours as outlined in the DFCC SHE manual clause No
14.1, 14.1.1 & 14.1.2.The administration team makes available in the control
room to assist the MIC.

11.3 Responsibility of Emergency Rescue Teams:

 Fire Fighting /Rescue Team: Members of Fire fighting/Rescue team are


trained fire fighters & good swimmer having knowledge to tackle various type
emergencies as stated above. They are able to rescue the person drowning in
the river water using boat. The boat contains Life jackets, Lifebuoys, Nylon
ropes, Emergency light &Torch ...etc.
As soon as team leader gets order from Site Incident Controller (SIC) the
rescue team rush to the emergency spot & starts rescue operation. The team
will continue the efforts until emergency brought under control. The effected
person shall be shifted to occupational health centre. The motto of the Rescue
Team is “Save the Life First”.
 Engineering Team: Engineering team leader gets “Switched Off” the power
supply in order to control the emergency as soon as he get message from the
Dy. Site Incident Controller. They provide all the technical support to control
the emergency such as restore power supply using DG set, provide lifting
equipments, tools & tackles, required drawings, PPEs… etc. The team leader
will arrange vehicles for transportation of the manpower & material.
 First Aid Team: The Doctor will give first aid treatment to the injured
person / victim. To communicate localized hospital for further medical
treatment & Ambulance if requires. His main function is to give first aid till
the victim gets external medical help. The Doctor will provide Cardio
Pulmonary Resuscitation (CPR) to the rescued person immediately. The
Doctor will make available in the Occupational Health Center located in the
Near Store with assistant medical officer.

 Administration team- Administration team leader will arrange all the


welfare activities required at the time of emergency. The team leader
communicates with hospital, fire brigade, police station for external help if
requires after getting permission from Main Incident controller (MIC). The
team will act as supporting roles to MIC & make available in the Control Room
with MIC at the time of emergency.
 Security team-Main function of the team is to cordon off the area or site and
controls the movement of the outsiders as soon as emergency declared. If
police help is required, the team leader will contact the Admin. team leader.
The Admin. team leader consults the MIC & get permission for the same.

11.4 ASSEMBLY POINT:

At Bridges no -92 “Assembly Point” is designated & board is visible. It is intended to


accommodate the workmen at safe place. They may be evacuated if emergency
becomes out of control.

11.5 EMERGENCY CONTROL CENTRE:

The Deputy General Manager office will be used as Emergency control centre at
office area for controlling emergency. The emergency control centre should consist
of the following documents & facility.

 On Site Emergency Plan


 Site Map
 Important Telephone Numbers
 Torches
 Communication facility.

11.6 OCCUPATIONAL HEALTH CENTRE:

The occupational health Centre is established at Near Store for giving First Aid
immediately to the victims. The occupational health Centre consists of First Aid Box,
Medicines, Equipments like Stethoscope & B P Apparatus, two beds are there for
patients & Emergency vehicle. The Ambulance will be provided by the tied up
hospitals. The list of tied up hospitals, Address & Contact numbers of the doctors …
etc. are available as stated below. The medical assistant is appointed & available
round the clock. The First Aid Box is allocated to all sites.

Note: Patient will be initially treated by Dr. Nilesh gavad, and further will be referred
to Sanjeevani Hospitals, Virar, if required.

11.7 RESCUE EQUIPMENTS & COMMUNICATION SYSTEM:

 1. Floating Platform (Barge) 2.Tug Boat 3. Life Jackets- 10 Nos. 4. Lifebuoy with
Rope -10 Nos. 5.Nylone Ropes, Blankets, Hand gloves & Socks, PVC Sheet, Aired
sleeping bag, Paddle,04 nose, Emergency Torch Emergency vehicle ….etc., are
available at Bridge-92 for rescue of drowning person.
 Fire Extinguishers are available on the Gantry & near electrical installation for
combating fire emergency..
 The Siren is installed for workmen & general public information. When Siren is
wailed for half minute twice it indicates that emergency caused at site.
Everybody has to be alert & all hot work should be stopped immediately. All
workmen shall assemble at assembly point.
 When emergency is over again Siren will be wailed for a minute continuously.
 The mobile phones are issued to the responsible person for communication.

12. ON SITE EMERGENCY PROCEDURE:

To combat emergency in time, good coordination is required between field staff &
controlling authorities. The Rescue Team members’ quickness & competency play
crucial role hence mock drill to be conducted at certain interval for enhancing
awareness & skill.

As mentioned in the emergency rescue flow chart as soon as emergency situation


arise, the Site Incident Controller (SIC) will inform to rescue team leader &
engineering team leader for strike on the emergency spot. Later on he will inform to
the Dy. Incident Controller (DIC) to pass on the information to the authorities. The
Site Incident Controller will isolate the emergency spot & orders workmen to
assemble at assembly point. Later on guide the rescue team for combating & Simul-
timeously procures requirements for combating emergency from the engineering
team. The SIC will concentrate on control the emergency situation only &
communicates with Dy. Site Incident Controller to shift the injured person to the
Occupation Health Center (OHC) for the medical treatment. The SIC will continue
efforts until emergency comes under total control.

The Dy. Site Incident Controller (DSIC) will pass on the message to the Dy. Incident
Controller. & Security team leader to cordon off the main access & control the traffic.
The DSIC takes care of the workmen, assembled at assembly point & restricted their
movement & take stock of the headcounts.

The Dy. Incident Controller will rush to the spot & assess the emergency situation &
guide the Site Incident Controller if requires. He will report to Main Incident
Controller (MIC) & gets suggestion for combating emergency. The Dy. Incident
Controller will act as coordinator between Control room & emergency site.

The Main Incident Controller (MIC) controller will make available in the control room
& gets update information from the DIC & controls the entire emergency
management. The Administration team will rush to the control room & assists the

MIC. The Admin. team coordinates with Security team & external agencies
simultaneously.

 The MIC takes decision for involving external agency like Disaster Prevention &
Management Center (DPMC) / Fire Brigades, Police & District Magistrate/
Collector if the severities of the emergency increase & going to affect the general
public living in the vicinity of the project site & mitigate the consequences.

 The MIC reports to DFCC authorities verbally & followed by a written report
within 24 hours after emergency gets under controlled.

 The MIC will visit hospital to take care of the injured person if human injury is
caused.

 The MIC is responsible for preparation of the investigation report. The MIC
constitutes the investigation team & under his leadership visits the site for finding
out the root causes of the incident. The MIC involves himself in the investigation
& gets the report prepared. The MIC will review the investigation report with all
rescue team members on the table to get their views/suggestion before
finalization of the investigation report & finally the SMIC submits the report to
the DFCC after emergency is over.

 The MIC is authorized to address the press & NGOs.

13. MOCK DRILL:

The efficiency and effectiveness of the On Site Emergency Plan can be checked by
conducting the Mock Drill. The following procedure shall be adopted.

1. The Manager-SHE (Fire) initiates the Mock Drill on quarterly basis.

2. Assemble the MIC, DIC, SIC, DSIC & all emergency rescue teams members in the
conformance room. First conduct Dry Mock Drill as follows: The Sr. Manager-SHE
explains strategy & importance of the Mock Drill on the board. The Sr. Manager-SHE
reminds responsibility of the team leaders & members. Ensure about equipments &
communication system. Gives their suggestion to finish off the Mock Drill in short
time means combat emergency in time.

3. Observer: The Senior management personnel can be designated as observers.


They should observe the onsite emergency management and note down the
response time, response of the Rescue Team members.

4. Fix the date for the rehearsal at the Bridge No: 92 “Switch On” the Emergency
Siren for half minutes twice any time & declare the emergency.

5. The Site Incident Controller (SIC), Emergency Rescue Teams, Dy. Site Incident
Controller (DSIC), Dy. Incident Controller (DIC) will come in action as per their
assignment and start rescue operation.

6. The Main Site Incident Controller (MSIC) & Admin. team will rush to the”
Emergency Control Room”& start the function as per their responsibility.

7. All the teams should act promptly for controlling of emergency as per their
function until emergency is declared over by wailing siren continuously for a minute.

8. Observers will note down the time & deficiency.

9. Members of the emergency management will be assembled again in the


conference room. The observers pointed out lapses & remedial suggestions for
improvements.

10. Requirements for compliances of the deficiencies will be initiated by the SHE
dept. & improve the system.

11. Records to be maintained & present when DFCC & Government authorities asked
for it.

14. TYPES OF EMERGENCIES AND RESCUE:

There are three basic types of emergencies:

1. Operating Emergency
2. Fire
3. Disaster

 Operating Emergency: It is an emergency that can be locally handles by


unit personnel without external help e.g. small fire, gas leaks, power failure …
etc.
 Fire Emergency: It is a fire which is beyond control of operating staff or the
unit. It requires the assistance of the external fire fighting force.
 Disaster: It is resulting from natural phenomena like earthquakes, volcanic
eruptions, storm surges, cyclones, tropical storms, floods, landslides, forest
fires & massive insect infestation. Disaster is catastrophic situation in which
day to day patterns of the life suddenly disrupted and people are plunged in
to helpless situation.

15. Method of rescue operation:

15.1 Emergency In case of Fire:


This is type of emergency will be handled by Rescue Team/Fire Fighting.
The Information is disseminated by wailing Siren. The Site Incident
Controller (SIC) will control the fire emergency by activating Rescue
Teams. The Extinguishers, Sand buckets kept are at conspicuous place at
the Bridge: 92 will be used by the Rescue Team for extinguishing the Fire.
The Diesel pump is also installed for pumping out water from the river in

the case of major fire. If fire emergency becomes out of control external
agency help will be taken.
Incident resulting in a minor burn injury can be treated by the by
Occupational Health Centre located In the Near store. In the case of severe
burn injury, victim will be sent to hospital for medical treatment.

To avoid Fire & Explosion, the fire prone materials are collected during
housekeeping every day. The flash back arresters are provided to Gas
Cutting sets, both ends of torch & gas cylinders (LPG & Oxygen) to avoid
back fire which generally resulted in explosion. Two hospitals are
tied up for the emergency help. Contact numbers & Address are stated
above in point No: 11.6.

15.2 Emergency in case of fall of person in the River water :


This type of emergency will be tackle by the Rescue Team & First Aid
Team. The rescue boat is anchored at landing point near bridge no: 92,
contains Life saving jackets, Lifebuoys & Nylon ropes. Torch…etc. The
Rescue Team members are good swimmer & trained person for rescue the
drowning person from river water. They will start rescue operation as soon
as get message for the drowning emergency. The life saver chooses the
safest way to rescue the drowning person.
1. He has to remember to Reach & Throw methods. Don’t enter into the
water unless it is absolutely necessary. Stay on land & reach out with your
hand or a sick /branch, throw a rope or a float. Only trained life savers
have to swim & tow the unconscious person to dry land. If the person is
unconscious, carry him with head lower than his chest once. He is clear of
the water to minimize risk of vomiting.
2. Shield the rescued person from the wind if possible, to prevent the body
from being chilled any further & preserver body heat too. This is known as
the wind chill factor or preventing from Hypothermia.
3. Treat the rescued person has follow:
3.1 insulate the person with extra clothing or blankets & cover his head.
3.2 Take or carry the person to sheltered place as quickly as possible.
Don’t put him on ground directly, Keep him on plastic sheet or blanket.
Don’t leave person alone someone must remain with him at all times.
Give warm drinks to the rescued person for rising body temp.
3.3. The First Aid Team Doctor will attend the rescued person immediately
& eliminates inhaled water from his stomach. If rescued person
becomes unconscious, open the airway & check breathing, If requires,
Cardio palmary resuscitation (CPR) treatment will be given by the
Doctor. The Doctor will decide the seriousness of the case & shift the
rescued person to the hospital for further medical treatment.

Safety Point of view permanent hand railing & toe guard are provided on the
Gantry to protect human falling in the river water. When the Gantry will be
shifted on the river water for piling work the Safety Nets will be provided to
make up open space between two Gantries to detain person falling in the river
water. The Lifebuoys (PVC Ring) will be kept on Gantry platform & kept in

floating condition in the river water surrounding Gantry so that falling person
can pick up lifebuoy immediately. Daily attendance register will be maintained
for the workmen & staff. Training will be conducted for workmen & supervisor
those are working on the Gantry platform.

At the time of high tide VAITARANA river water level reaches about 11 meters
& in normal condition river water remains up to 5 to 6 meters.

15.3 Emergency in case of collapse of lifting appliances and


equipment dropped in the River water :
In this emergency case materials will be lifted with the help of the tripod &
winch or Crane, Hydra using lifting equipments, tools & tackles…etc. The
rescue team divers will be guided by the SIC and Engineering Team leader.
The floating platform (Barge) can be used if it is required. External agency
help can be taken if our rescue team is unable to tackle the situation.

15.4 Emergency due to spillage of oil or construction material in


to the river stream:
The Diesel is for the DG sets which are located on the Gantry. The MS tray
is provided for collection of the spillage. We are not using oil in much
quantity even though oil spillage is found from machinery or during
maintenance due care is taken to avoid any contamination of river water
or soil using collection tray & waste cotton cloths for absorbing / wiping
out the spilled oil & finally it is collected in carboy.

There are rare chances of falling of concrete & iron rod pieces in to the
river stream. The Supervisors (SHE & Civil) conduct Tools Box talks before
starting concrete of the piles to educate our workmen & remind them
hazards involve in concreting.

15.5 Emergency due to sudden release of water from different


rivers, canals & barrage …etc
The Bridge: 92 is being constructed on the river VITARANA. It is
downstream area of the river, meets with the Arabian Sea. At the time
high tide, back water of the Arabian Sea comes in the river but it does not
cross the danger mark. The irrigation dept. provides information in
advance for sudden release of water from the DAM. In such case
construction activity is postponed & equipments are demobilized at safe
place.

15.6 Other Emergency Situations:

 Collapse of A- Frame or guide frame or damaged of gantry.


 Natural calamities like Cyclone, Flood, and Earthquake…etc.
 Criminal or terrorist attack.

Incidents such as explosion, flood, typhoon, hurricane, earthquake & terrorist attack
causes multiple causalities & significant loss of property. It compels to prolong shut

down of all construction activities as general public routing life is also severely
affected. Loss of confidences of the involved personnel consume more time to
normalize the situation. In such cases we depend on the Local government
administrations & District disaster management cell help to overcome the situation
