A. DETAILS OF NON – CONFORMITY
We have already given strict instructions to all supervisors that hot work or confined space work cannot be
allowed without approval from the HSE department. Your team was conducting brazing work with Hydrogen (H2)
gas without informing the HSE team. Although our team was inspected during the work, your team did not take
any safety precautions, and we had to stop the work. Afterward, without our permission, your team proceeded
with the brazing work. This is unacceptable and we cannot compromise on safety regulations.
